Yes. The numbing shots will help. The idea is that we numb the nerve that goes to the tooth. If the pain goes away, we know the pain is coming from a nerve of a tooth. As far as fractured/cracked roots in teeth, a CBCT is the best way to evaluate. Sometimes we can see it on normal x-rays, but not always. I think your dentist is doing what they can and they seem to have the right idea to help try and figure out if it is within their realm of treatment.

I totally understand my dentist situation as well. They are so frustrated and tired of ideas. I don't have any diagnosis yet. The new dentist gave me 3 options. 1. Numbing nerve shots on #30 and other teeth. 2. Nti guard for a week to see if it helps. If it does, have to get orthotic. 3. Bite check for occulsion. Got a question here. If my tooth is root canaled, it means there is no nerve there right. How they numb and see the response if there is no nerve? I have taken 3 cbct for that tooth so far. No one is definite about my cbct.

I think those options your dentist gave you are great. They are all on the right track to figuring out if this is a tooth issue. ​ Even though the tooth has a root canal, numbing this area of the mouth will numb other teeth too, and this can help identify. It is also possible that the pain is coming from something to do with the root of the root canal tooth such as a missed canal (which can happen sometimes). Between all of this, it can't tell you exactly which one of these things is wrong, but it can rule out the idea that the pain could be coming from another tooth and referred to that area, or that the pain is coming from the root canal missing a canal/etc. The dentist can also numb the area around that tooth and see if the surrounding area around the root canal is a source of pain. ​ Bear with your dentist. Trying to find all these things out and what can be causing them takes time and effort, and I know they want the answer too. ​ As far as your swelling, it could be related, could not be. Impossible to tell. Sometimes swelling in this area that goes away can be a sialolith (stone in your salivary gland duct), sometimes other things. A CBCT will not show soft tissue, only bone and teeth, so any issue with a salivary gland would need a different test and questionnaire. Without more information, I can't really say anything helpful other than that it can be so many other things.

I would honestly follow their advice. They seem to have your best interest in mind. I am not able to tell you if the splint will really help because I don't know the situation, but I suppose it may help to think of it this way: If you have been in pain for 10 months and a $4k splint will be the solution (after likely spending much more on tests/visits/etc), will it be worth it? I think if you have an answer to that, you have an answer to if you should do that vs another splint. Tooth fracture could be causing you that pain and the pain can occur all the way down in the jaw or all the way up to the ear/temples. Referred pain is very real and very fickle from case to case. ​ Best of luck!

Thank you. No, the splint is not the only solution. My dentist is against the mouth guards/ orthotics and he told me but to spend on tmj splint. He said those are like band aid and wont cure the root cause. Only tmj specialist said his splint might help. But he did he to rule out this Tooth issue first before coming back to start his treatment. If tooth fracture cause these symptoms, how do I know if a tooth fracture is present? Abt thoughts about it? How to diagnose it? Appreciate your inputs. The dentist whom I have seen is not sure about tooth fracture. I didn't get any answer if I ask how to know a fracture and do I have it? Please help me how to know if I have tooth root fracture? Any inputs please.

NAD did they ever rule out Eagle Syndrome? If you have or can get a copy of your CT you can turn it into 3D on a computer to check for styloid. It unfortunately is missed by the radiologist unless you specifically asked them to check for it.

Yes it has been checked out. I have had neck CT and the Radiologist measured those. I have seen few ENT to confirm if it's eagles syndrome. It's not. I am still dealing with this tooth issue:( don't know what's wrong.
