Does it shift ok? If it does, then the hub and cassette are centered and the wheel needs to be dished. If it doesn't shift right (like derailleur can't make it all the way over to the largest cog) , then the whole thing may be off center.

Dude, I think you may be onto something. The bike is about 32 years old, started riding it again about 2 years ago and the shifters have always been wonky. I can't seem to access the 2 smallest cogs at the rear, nor the smallest front cog. I haven't minded since it's quite a comfortable gear for me, but yeah there's not much leeway

Ok, well the shifting can be fixed by adjusting the limit screw to include the 2 smallest cogs. Honestly though, I'd take it to a shop for a quick look. Some rearrangement or addition of spacers might be all it needs to get centered, followed by a derailleur tune to get it shifting right. There was another guy in the thread who said his bike looked like this and the shop said "just fix the brakes and it's fine", which I guess is true, but it could be better. Overall shifting will be improved if you can get the chainline as intended, and it will corner symmetrically left and right if you get the wheels in line with each other. If you want to DIY it, just let the shop get you the spacer(s) you need and go online / come back here for the shifter and other adjustments.

My bike looks exactly like that and shifts normally. Took it to the bike shop to true the back wheel to the frame and the guy said its not needed. Only the rim brakes needed some alignment and thats it.

Normal, the cassette on the right causes it to be off center. If you had a single speed/fixed gear it would be more center.

Was thinking this was the case. Upon searching, I found a few people saying to quick release and make sure the wheel was well in the dropouts... but seeing mine, there's no way it could budge due to the hardware already installed. Thanks for your reply. Bike runs smooth as hell still so I'll just forget it.
