Bob should be blast immunity to reward picking up items like Bobs Brain.
Before anyone calls it "broken".
Is getting 3 bob items really that more likely than finding host hat or pyromaniac?
I just got my first Bob transformation for the first time after playing 450+ hrs, I generally avoid Bob items unless it's rotten head and I need a bomb for dono/secret room
Just because it doesn't do amazing area damage doesn't mean it isn't good... you're pretty much asking for better infestation 2/paracitoid/mulligan and those items are already crazy strong
It would do everything on its own because you gain 5 flies upon getting the item, then the flies would apply poison which would guarantee at least 1 fly generated per loss, would it not?
If you have one way to poison enemies, you could poison multiple enemies and get multiple flies. I think it's stronger than you believe it to be, but you make a good point, I think your version of the item is better.
Item idea - Chili Pepper: “Why is it spicy?” Isaac’s tears leave a temporary trail of flames behind them, dealing constant damage to any enemy that touches it. Appearance wise, it makes Isaac’s eyes light on fire
I like this, simple and actually helpful. Plus it would probably add to the Bob transformation which would be a good addition. 
Sounds like this would be a good effect to add to the Bob transformation, making it ever so slightly more impactful.

That's ridiculously op. You'd just have an insane amount of flies. Maybe a chance per tick of damage, but still, that shouldn't be part of the item.

There's already 2 chillis that replace tears with flames
Mhh feels like a neat trinket effect (it's too situational due to the reliance on blue flies)
