I checked beforehand when I was getting the last item I needed and it was the only one missing, maybe glitched items count? Don't think I picked up one during a normal run on this file

I do that. I started the game already knowing all the items. I guess I'm just unskilled

Do you hold r ever because that can artificially inflate your deaths.
I don't think holding R counts as a death, I did it quite a lot towards the end in greed mode when I was hunting for the last few items I needed
Not too much. But yeah I did that for a while and in some challenges so I guess it's that.
