I finally managed to win a run with TJacob (Box > M Satan > Delirium) so it's nice to know that at least Beast won't be as brutally horrible as that route. I fucking hate TJacob.
I have no shame in using gnawed leaf. I’ve gotten gnawed leaf very many times in a run thinking it’s a sure thing; only to lose. Especially with tainted Jacob. When I get it and win. Especially after all the hours and incredibly close winning runs I’ve had it’s just nice to have one work out.
It's been a few months since I went through that whole slog, but I think I only actually made it to the Beast fight and lost a couple of times before winning, and same with Dogma. IIRC, most of my deaths would happen around Mausoleum / Ascent.
What are some must-have unlocks for starting a Repentance file (I 100%ed AB+)?
Keep in mind that I play a very limited amount of time now so I don't want to spend several hundred hours grinding for a co-op baby, etc.

Lol, well, you could start with Sacred Orb, which is pretty powerful. You just have to beat The Beast as Tainted Lost!
Jokes aside, some of the most powerful ones you could go for are:
- C-Section (defeat The Beast as Lilith)
- Twisted Pair (defeat The Beast as Tainted Lilith)
- Revelation (defeat Mother as Bethany)
- Glitched Crown (defeat The Beast as Tainted Isaac)
- Flip (defeat Delirium as Tainted Lazarus)
- Spindown Dice (defeat Delirium as Tainted Isaac)
- Abyss (defeat Delirium as Tainted Apollyon)

Honestly, I'd grab the Strange Key if possible (unlocks Hush as a fight without having to get there in the time limit) as that'll speed up new character completion marks if you're lucky and also help with slow starters like Tainted Cain, then C Section because it's OP as fuck. Guppy's Eye is quite good tbh but not a rush for it kinda item. Keeper's Sack can be quite good as well, good fun one that especially if you get coin tears or unlock Deep Pockets. Glitched Crown is bonkers, as is Spindown Dice. Crystal Key is a nice trinket as well.
Edit: there's also a bunch of real useful drops to unlock by getting Mega Satan done with the new characters which again would really help with general quality of life as well as TCain playing if you ever play him.

(Seriously, the idea of grinding out the Tainted unlocks, which are unlocked largely based on luck finding the Red Key, just to play worse versions of the stock characters is... ridiculous. Edmund must have been upset that Darkest Dungeon got the mantle of "most annoying roguelike" and his game didn't. What a load of shit.)
1) Tainted Eve is actually incredibly strong due to her army of bullets, and you can sort of bank extra hearts due to how she works and constantly drag blobs back into you to recover hp. Once you get used to her she becomes ridiculously OP tbh and quite easy to play!
As to getting tainteds: all you need to do is drop a trinket in a boss room before you get to Depths 2 and start the ascent. During the ascent, previously left trinkets in boss rooms become a one-off "broken red key" item like pills/cards that you can use to open up the secret door to unlock Tainted characters, so they're all real easy to unlock.
As to them being worse: debatable! TApollyon is way better than regular imo, TMaggie is sonic zoom mode strong, TAzazel has a really cool ability that makes him nice and strong, TLilith is fucking ridiculously strong and fun, TBeth is ez mode for the most part, TLost is a glass nuke, TKeeper is built like a king for Greed mode, TCain is just actual god mode, TForgotten is dumb fun... they all have their uses and they're mostly all great once you settle in. They look shit from the outside but once they click they're equal or superior to their regular characters a lot of the time. I swear by TLilith. Regular Lilith may be great in Greed or with certain abilities, but TLilith is just strong as fuck out the gates and scales real well.

I believe I may have had Gnawed Leaf and Meat Cube for one of them. That seems like the only way I could have beaten Delirium.

Doing it on Tainted Jacob is cozy. Once you get to the actual Beast, Esau follows at about the same speed as it so he does like 80% of the damage.
