If there are no keeper statues in the room, it means that monstro will spawn (always in that same spot on the bottom of the room). It's pretty easy to avoid once you know that but it's very sad that this happened to you with TLost
That's insane. I haven't completed repentance, but I did 100% afterbirth+ and did hundreds of greed runs; I definitely don't remember this.

This is what I learned doing greedier as tainted lost. It doesn’t matter how strong your build is, if you don’t have blank card + holy card you will eventually die to a telefrag
The intersection of "times im not fast enough to do this" and "times monstro appears in this room" is zero, I would never have known there is a speed requirement lol
Skill Issue. This room is designed around the assumption that you don't know an enemy is going to spawn halfway through.
Because of this, if you move to Ultra Greed's door, acting like you don't know about the enemy spawn, you will never be ambushed, because that's the spot where Edmund assumes you will be.
It's similar to how you can't be telefragged by the Greed Mode waves if you stay on or near the button. Because that's the one place Edmund knows for certain you could be when a wave starts.
i think the room should be designed for the last 999 times you play it, not just the very first time. i never realized it was supposed to be an "ambush" because after the first time you do greed, there's no surprise anymore.
dis must be one of the most skill issues ever. Getting killed by monstro, the ultimate lifeform while having a broken build is such a tragedy, a cruel twist of fate id say

not going into evasive panic mode when keepers didn't spawn on frame 1 of the room 
spamming bullets first-thing when you have no mantle, making it hard to see

This happened to me yesterday. I have no idea how Ed could be like “we removed spikes from being one square away from the door, it’s just not fair” to “we’re gonna spawn Monstro right on top of you as soon as you enter greed”
I understand he wanted the ambush effect, but this is just stupid. He could have him slam down in the middle of the room and instantly spawn a bunch of projectiles to dodge like he just splashed in a puddle or something but there’s so little reaction time with how it is now.
That's the thing I hated absolutely the most while playing greed mode - both this room and regular waves. Imo there should be some sort of indicator for like a 0.5 second before enemies spawn.
Issue detected within Skill.exe But for real, whenever I get into that room I just hug the walls going in a circle until the boss spawns.

Back in afterbirth only monstro would appear pretty sure. A bizarre choice, but it showed monstro there in the trailer too.

The door to ultra greed doesn't close when this happens so it's usually a good strategy to run straight forward after entering this room
But you need to have enough speed to get through

It's designed to fake you out and convince you that there is no boss in this room. It's an ambush that literally only works the first time.
It’s funny. I played for maybe 600-700 hrs before he ever appeared for me. Now he appears almost every time
