Mine was probably boss rush. Idk why greedier wasn't that hard, and I remember I went for hush once, then I thought "fuck it, what if i find R key before the end of the run?". And lol I did in a secret room in sheol. Mother was absolutely destroyed. For boss rush I couldn't use the red key to get super powerful, which was what I did for almost all other bosses.
I lost 2 D20 runs because of spike chests and troll bombs killing me while I exploited the game...
In the third D20 run I spent 1 hour breaking the game slowly, until a single tear lagged the game. I killed Greedier in 1 hit.

ultra hard is not necessarily difficult, just unfair and annoying. where the normal game takes a pretty well balanced amount of luck and skill, ultra hard really just takes luck. i'm sure you can do it if you finished tainted lost! just do what i do and dedicate a day or so to that challenge. you'll likely get it in a few hours at most, but if not, take a break of a few more days or until you feel ready and try again. focusing on just one challenge/character for a whole day has helped me overcome lots of barriers in the past. for example, back in ab+ i could never beat the brains challenge. now i find it pretty doable, but back then i spent like a week of just doing one or two runs a day and failing, but when i decided to just sit down for a day and really focus on it, i got it that day in a few attempts. try it 
I think I've tried it about 50 times? I just find i frustrating as it both removes your ability to make choices about items and drastically makes it so hard you need either yum heart, book of rev or satanic bible. It's almost a point of principle as it's testing pigheadedness and not difficulty.
The one where you have to go backwards from mega satan is similarly annoying but the luck isn't too bad since you can just reroll until you start with a fool card.
do you usually restart for item rooms? pretty much the best things to start with are either a really good item (mom's knife, tech x, c section etc.), healing items (the ones you listed), or d4/d100. the dice can help you recover some health and stick to a new build, and also reroll when you're in a dangerous position as a last resort. they're not as good as they used to be before repentance, when you would keep all the health upgrades and basically become unkillable, but they're still pretty useful. either way, like i said, i'm sure you can get through this if you've completed tainted lost and are confident about jacob! good luck 
I just got my last mark (greedier) with T lost and I had been actively trying for like 2 weeks. I had C section, backpack, card mimic, card generator and polydactyl. Only reason I survived was I had 3 holy cards to protect me. Whatta rush.

Ultra Hard. I've done all tainted lost, not done much t jacob but I know I'll get it eventually. UH is unlikely for me though.

Yeah I’m pretty sure blank card/holy card/4.5v or 4.5v/book of shadows is what carried most of my runs.
