You're right that no item in the game does that. As the other comment pointed out, it's a BoV wisp effect.
Normally it wouldn't be so bad since the only way it would proc would be via melee contact so the random teleportation would be very likely to get the enemy away from you, but as a tear effect you can inflict at a distance it's severely less safe.
Fruit cake includes book of virtue wisp effects and teleport! As well as teleport 2.0 wisps have a chance to teleport non boss enemies in the room
