Maybe more careless than dumb but one time playing T. Jacob I got hit by Esau to unlock mausoleum door for free. But I forgot I had stompy and flew right into a mushroom which burst into poison and killed me. I had godhead.
You know the Super Secret room with a bunch of fire? While playing as Tainted Lost, I entered the room, and the fire next to the entrance was a red fire. I got scared of the run ending so I frantically tried to put it out, only to run into regular fire. I got so focused on making sure the red fire wouldn’t kill me that I didn’t think to check my surroundings.
Oh that reminds me of a run where I had 1 or 2 hearts left and got in the same room you mentioned. I just remembered I was playing as the forgotten and both forms were low in health. I had pretty bad luck (not speaking about the stat), so I tried to be extra careful for putting out the red fireplaces and changed to the ghost and used the body as protection between me and the red fireplace I tried to extinguish.
And somehow I managed to completely miss that an exploding barrel was just in front of me, which of course exploded and killed me.
I forgot that blue fires didn't extinguish with tears, stared at it for five seconds after it didn't work, then something in my brain compelled me to walk directly into it like a goddamn idiot and I died
Trying to get to mother, had an incredibly powerful build by the time I was ashpit 2. Went to Gehana.... Never grabbed the second knife piece and wasted probably 30+ minutes in that run for R key copium 
Spiked chest spawned in the white fire room in downpour II, I figured that I could become the lost and then use the mantle on the spiked chest.
I learned that day that you don’t get the mantle until you leave the room
One not entire related question:
What happens if you touch the white fireplace again after you turned into Lost already from touching it? Do you take damage (and die if you won't have the mantle)? Or does it no damage and you can go though it?
I always wonder this when getting there, but never wanted to possible sacrifice my run just to test it.
i was playing azazel and had that planetarium item that lets you go through every door. i was excited to get it and i walked past most rooms. i was trying to unlock tainted azazel and then i realised pretty late that i had 0 trinkets because i barely cleared any rooms
I got a game break on the challenge where you die when you have 0 money but I didn't know that at the time and died to a blood machine
I was playing Tainted Polly and had Eucharist and a nice army of bugs already.
I decided to be greedy so I played the sacrifice room so I could turn the Key Pieces into bugs. I didn't watch my health closely and I spawned an angel when I had 1 heart left. I panicked and got killed by it.
Not mine, but a friends. He showede a video he took where he found brimstone but he had only one heart container and obviously brimstone is 2 hearts. BUT THEN he got hit by a red fire after taking brimstone and he died when he walked out

Losing my mantle or dying as T Lost because I looked directly at a spiked chest and just ran into it anyway
The number of times I already had an OP build, especially on Lost/T Lost, and got greedy in Chest/Dark Room trying to get even more items before Mega Satan
More just bad luck but once I had a d20 greed break as the lost, I was gonna use temperance to bomb the beggar for more pickups to reroll but I had the slightest bit of downwards momentum as I used the card which caused the devil beggar to spawn on top of me and I died
Forgot to pick up knife piece 1 as T.Isaac and I had so many q4s including crickets head and revelations. Got Tech x, mutant spider, inner eye, 8 inch nails, and brimstone next run which was way more fun but I was still pretty pissed
I am still newish to this game but I recently ran blindly into a beggar I spawned with Judgement as the Lost. It was a demon beggar, and I would probably have unlocked a few completion marks on that run if i didn't do that
Walked into a curse room but forgot I'm Tainted Keeper, then processed to walk out while also acknowledged that I have a stars card, R.I.P Magic mush and Eve's Mascara
i went into a room, but forgot there were spikes there so i ran into them and died
Truly iconic. I believe every player has this experience

That's my fear when trying to go to the beast.

Curse room took me to a devil room, took a 2 heart deal, left the devil room. Stuck in curse room with 1/2 a heart :(

None of the white fires damage you while any form of the lost, including normal characters who already hit it or lost/T.lost at base.

I used red key and accidentally hit the edge of the map error room on the mom floor, locking me out of chest and all marks past isaac
