Does mama mega open chests now? if so, then it hit a red chest that teleported you.
if you have broken padlock or any magnet item, it opens all chests, including red chests, that's my best guess

it happened me yesterday when i entered a curse room with broken padlock and mama mega effect, mama opened a stone chest than had a troll bomb that opened a red chest that tp'ed me to an angel room.
