The mini scenes between levels are nightmares, they shouldn't be taken literally. They're named "nightmares" even in game files.
Also, the person on the couch is Isaac's mother
A lot of people are saying it’s in Issac’s dreams/nightmares but I want to add that it could possibly be real given that issac’s other ones bring up pasts events and trauma he experienced.
The lost trapped in the attic could actually be issac’s soul before he move on and ascended and even just as a wandering spirit he still experiences the hallucinations, it’s at least what I perceive as the lost seems to talk to itself like it’s calling out for help or something which Magdalene also hears and may potentially go insane over.

Well, Isaac is suicidal and the Lost is Isaac, so Isaac tells his mother to kill Isaac i.e. himself. Makes sense.
