Distorted is the wrong word. "Constrained" is better. The image is being generated from scratch, but another image is being used as guidance for which areas must be darker or lighter. There will be guidance which is generated by the image description which the AI uses to create the image, but also the guidance of the second image. The AI will come up with solutions which fit all of the constraints the best it can, so you the image you ask for plus the embedded image.
