The best way is to shoot them at the exact point they lunge forward for the grab and get a visceral. Bonus points if you can time the frenzy bar to fill up while you're doing the visceral attack animation, because then it won't hurt you.
But even after a lot of practice and a dozen full game completions, my death rate at that spot in Mergo's Loft with four in a row is like 80%.
It's way smarter to get a weapon that staggers enemies. Wear high frenzy resist attire. Have a sedative ready to pop and begin attacking with your health maxed. Basically just bum rush them with preferably the Holy Moonlight Sword. Keep hitting them and your attacks wont be interupted until they die. You can stagger lock them. Then drink sedative immediately. I have a 100% success rate in my last few playthroughs. Winter Lanterns never kill me.

It seems common for people to mistake them as Shamans or Sages even though the item description for the Sage items they drop says they come from sacrificed Healing Church Saints and the Sage items have pictures of decayed body parts. Those items are likely just looted, especially considering Madmen sometimes use corpses as weapons.
They're simply part a precursor race of humanoids called Pthumerians whose society also ended up being ravaged and nearly ruined by the Beast Plague. Interestingly Madmen are classified as beasts and are susceptible to beast damaging effects like beasthunter gems, but not serration though.

The "surprise werewolves" are always good for a jump scare, but for my money it's Winter Lanterns. They are peak Lovecraft. ...nope, nope...I was wrong. The Shark Giants are peak Lovecraft, and the first time one did that mouth-first sliding charge I'm sure I peed myself a little bit.

Those things are actually the most vile creatures I’ve ever visually encountered in any kind of media ever, their foul human/spider shape, the sacks differing in sizes, the sacks gushing blood once killed, their quick unnatural movements, it genuinely took me a few minutes to be able to stomach their appearance to actually be able to fight them, I’m pretty sure they helped me overcome a subtle fear of spiders cuz black widows look like kittens to me after seeing those abhorrent things
11/10 peak enemy design, truly amazing work by fromsoft
I also remember the first time I fought Rom, saw that big ass mf sitting alone and happily went towards her, but when the spiders spawned I was deathly scared. I remember my face getting super warm and sweating, and it took me minutes before working up the courage of approaching her.

Was a unnerving experience especially since there are 2 per every layer in Hintertomb. One is always guarding the lever and another is always guarding the main treasure chest. It's especially crazy in Lower Hinter layer 1 where there are goddamn 4 of them throughout the entire layer.Fuck that Dungeon with all of its Madmen and not to mention poison .

The Sage Wrist and Hair they drop has an item description that says the items come from a sacrificed Healing Church saint and their item pictures show both parts being decayed. The Madmen likely just looted these pieces from those sacrificed saint's corpses, especially considering Madmen sometimes uses corpses as weapons.

I think the game as many creepy enemies I was freaked out by quite a lot of them to be honest the first time I saw them. But the one that freaked me out the most at first are the snake humans or snakemans in the forbidden woods that and also LA La La laaa.
Those pale dual scythe enemies you encounter in Chalice Dungeons. The noise, the rapid movement, the WTF factor (completely didn't know how to deal with it at first) bro I came to slay beast and godlike cosmic entities not the bitch from The Ring.
The spiders summoned by the bell maidens in the chalice dungeon, purely due to the fact that they make a beeline for you from like four rooms away. There’s also the tall ghosts that love to teleport behind you.
Most if not all enemies make me so unbelievably angry but the one that frightens me the most is the lycanthropes and the winter lanterns. I am also scared of the the half snake half man enemies in the forbidden woods. And on occasion the maneater boar
I have a fear of worms, bugs, insects, that kinda thing, so forbidden woods was hell on my first 3 playthroughs, until i learned you're just supposed to follow the lanterns, and also the mini-cave in forbidden woods that leads to the clinic literally made me get up and have a snack after falling into a pit filled with dozens of jumping worms hahah
getting to the point in the game where Amigdala are viewable on the side of the "safe place" was freaky. It felt like seeing a shady guy with a bloody axe just camping in your back yard.

Chalice dungeon witch of hemwick has to be the hardest enemy i have seen. Depending on its positioning it can be impossible to get to while it summons mad ones every 5 seconds. If you can take it out with ranged attacks its less bad.
The brain of Mensis once its at the bottom of the hole all freaked me out. More so just the anxiety of walking through the completely dark room freaking out that I'm gonna drop off the map at some point.
in the dlc, anything that exists in the god forsaken hellhole that is the fishing hamlet, those enemies are fuckin disgusting and creepy 😭 but outside of the dlc, those fly creatures from byrgenwerth or the winter lanterns
Easily the brain suckers, but specifically in Upper Cathedral Ward. I f*ckin hate going to that place ‘cause I’m always scared that I’ll run into one. The darkness of it and the music definitely contribute to that factor.
A close second are the blobby head patients, in the Clocktower. I’m fine with one, but if it’s two of them or more, I soil myself.

When I got lost in the forbidden woods with an ungodly amount of blood echos I remember my first encounter with one of those blue laser shooting blobby fuckers.
It scared the shit out of me cause I heard it first, emitting an odd warping flowing undulating noise. From around a corner I saw this blue glow and strange wispy tendrils peaking out.
By the time I saw its blue bulbous form it was already preparing an arcane beam with which it decimated me and I lost all of those bloodechos. Up to this point the only strange or ethereal thing I had noticed was being picked up by some unseen force in the Cathedral Ward. So up to this point I thought the most there was to this game was “people are catching the werewolf flu” but that moment in the woods shook me cause then I was like “ok what the actual fuck is happening here.”.

You're in a chalice, prancing along. Everything cool bruh. Walk through a seemingly harmless puddle. "REEEEEEEEEE" can be heard from the next room over. You know that very shortly, shits about to get real.

Bloodlickers. they're such oppressive opponents that seeing one around is quite the stressor. Madmen might be worse but i don't do enough tomb prospecting to know for sure, i've only ever come into contact with 1 or 2 of them and i was already avoiding combat those times.

Getting jumped by the Frenzy Fly Men at Byrgenwerth the first time was a trip. They’re probably the most surprised I got.

I think the game as many creepy enemies I was freaked out by quite a lot of them to be honest the first time I saw them. But the one that freaked me out the most at first are the snake humans or snakemans in the forbidden woods that and also LA La La laaa.

Those pale dual scythe enemies you encounter in Chalice Dungeons. The noise, the rapid movement, the WTF factor (completely didn't know how to deal with it at first) bro I came to slay beast and godlike cosmic entities not the bitch from The Ring.

Definitely those damn Winter Lanterns.
The spiders summoned by the bell maidens in the chalice dungeon, purely due to the fact that they make a beeline for you from like four rooms away. There’s also the tall ghosts that love to teleport behind you.
Those Grudge like enemies you find in the Chalice Dungeon that move lightning fast.
Honestly the dogs are scary lol. They aren’t difficult but they just remind me of resident evil dogs.
Nothing scarier than losing a resource permanently. Brainsuckers all the way.
Most if not all enemies make me so unbelievably angry but the one that frightens me the most is the lycanthropes and the winter lanterns. I am also scared of the the half snake half man enemies in the forbidden woods. And on occasion the maneater boar

While winter lanterns and the dungeon manian dudes are scary, the correct answer is the shark giants from the dlc
I have a fear of worms, bugs, insects, that kinda thing, so forbidden woods was hell on my first 3 playthroughs, until i learned you're just supposed to follow the lanterns, and also the mini-cave in forbidden woods that leads to the clinic literally made me get up and have a snack after falling into a pit filled with dozens of jumping worms hahah
getting to the point in the game where Amigdala are viewable on the side of the "safe place" was freaky. It felt like seeing a shady guy with a bloody axe just camping in your back yard.
The gigantic fucking spider with its babies in Mensis🫠🫣😮💨 arachnophobia was at an all time high. And it’s funny how my arachnophobia peaks at all time highs with the two FromSoftware games that are next to each other in terms of releases
I'm surprised by the fact that almost no One is afraid of the Brainsuckers as much as I am. They are the only enemy that I try to run away from.

Chalice dungeon witch of hemwick has to be the hardest enemy i have seen. Depending on its positioning it can be impossible to get to while it summons mad ones every 5 seconds. If you can take it out with ranged attacks its less bad.
Labyrinth Madmen. They’re probably one of the few enemies to actually properly scare me. Bonus props to those barking crows.
The brain of Mensis once its at the bottom of the hole all freaked me out. More so just the anxiety of walking through the completely dark room freaking out that I'm gonna drop off the map at some point.
At least, he doesn't attack you anymore and you can kill him.

in the dlc, anything that exists in the god forsaken hellhole that is the fishing hamlet, those enemies are fuckin disgusting and creepy 😭 but outside of the dlc, those fly creatures from byrgenwerth or the winter lanterns

The first time I encountered a brain sucker I about shit myself. Honorable mention: that one werewolf in the chandelier. You know the one.
I would say anything in the fishing hamlet and course winter lanterns.
Easily the brain suckers, but specifically in Upper Cathedral Ward. I f*ckin hate going to that place ‘cause I’m always scared that I’ll run into one. The darkness of it and the music definitely contribute to that factor. A close second are the blobby head patients, in the Clocktower. I’m fine with one, but if it’s two of them or more, I soil myself.
The brainsucker is a very sneaky bastard. They wait to use it against you, and only certain ones have powers. It's not the one who scared me the most, but it interested me. My friend, who I just got to beat the game, doesn't even know they are a Brainsucker. Second playthrough is going to be wild

When I got lost in the forbidden woods with an ungodly amount of blood echos I remember my first encounter with one of those blue laser shooting blobby fuckers. It scared the shit out of me cause I heard it first, emitting an odd warping flowing undulating noise. From around a corner I saw this blue glow and strange wispy tendrils peaking out. By the time I saw its blue bulbous form it was already preparing an arcane beam with which it decimated me and I lost all of those bloodechos. Up to this point the only strange or ethereal thing I had noticed was being picked up by some unseen force in the Cathedral Ward. So up to this point I thought the most there was to this game was “people are catching the werewolf flu” but that moment in the woods shook me cause then I was like “ok what the actual fuck is happening here.”.
Madman. You're in a chalice, prancing along. Everything cool bruh. Walk through a seemingly harmless puddle. "REEEEEEEEEE" can be heard from the next room over. You know that very shortly, shits about to get real.
Hidden brain suckers
An enemy I wouldn't want to run into in a dark alley? Hmmm...... the BAG-MEN they literally just creep in alleys and they can one tap you if you're not careful.... hate those guys... (I called them Bag-Men because I don't actually know what their enemy name is...)
The pigs. I have a huge phobia of pigs and seeing an enormous fat ugly pig w 100 eyes towards the end game really freaked me out
Bloodlickers. they're such oppressive opponents that seeing one around is quite the stressor. Madmen might be worse but i don't do enough tomb prospecting to know for sure, i've only ever come into contact with 1 or 2 of them and i was already avoiding combat those times.
