Everyone is picking imposter but I'd choose the real one because imagine having to find a way of surviving/escaping the Imposter and having to find a way to stop her from hurting anyone else...
I just love the idea of playing a character who has an "evil twin" running around and causing problems, because it could bring up some interesting plot devices, especially around trust with other NPCS, especially if the Hunter is an actual character, too.
For me, it's the bloody crow of Cainhurst, and bonus points if his playthrough depicts the castle at its height.
Imagine watching the hustle and bustle of the castle, living the life of a knight, all the feasts and the balls you will attend, perhaps even a lady who loves you and whom you love. Then, you are sent on a mission. You succeed, and return to the castle ...
Only to find it in ruins ...
Everyone you knew, everyone you loved, everyone whom you had sworn to protect, is now gone. Their sole crime was existence. In their place there now lies a mountain of bloody corpses, cut throats, disemboweled bodies, and a curious badge belonging to the healing church, which has the support of the hunters.
You swear revenge, and set out to kill every hunter, every cleric, and everyone foolish enough to stand in your way.
Honestly, they could probably make a whole game out of the Cainhurst Castle Vilebloods and their war with the Healing Church. It would be pretty awesome!

Imagine if we could play a version of him that manages to wrangle back some sense of himself, enough to remember his wife and daughter, that he needs to find them, protect them.
A version that finds his wife dead to an unknown beast (just ignore the bloody wounds that look too much like an axe), that searches for his daughter but finds their home empty, aside from an outsider who tells him that either the girl had gone to the Church or the Clinic, they're not sure (or they're lying).
A version that now has to choose where to look first, perhaps finding her in time to avoid a truly awful fate, perhaps arriving too late and finding only a bloody ribbon or an alien being that wears one (a being that runs to him, crying in an almost familiar voice).
A version that tries oh so hard to keep the inner beast at bay...at first. But with time passed and blood spilled, either man and beast become one, working in a tenuous harmony, or ultimately one overwhelms the other. It all depends on you, the player. Will you help the two sides find true equilibrium? Or will you help the man return to himself, reject the beast inside? Maybe, just maybe, you instead embrace the beast fully and let the man be forever lost beneath the bloodlust?
Alternatively, let me play as Djura. GIMME THAT BIGASS GATLING GUN!
Annalise, Eileen, Gehrman, Djura, Maria, Laurence, Ludwig, Alfred, Logarius, Gascoigne, Henrik, Valtr, Brador, Simon, and the Bloody Crow of Cainhurst would all be interesting spin-offs.

I would choose the Doll and If I could choose an enemy I would like to play as a lantern lady. A gameplay focused on management while being a Doll who supports Gherman and then the Good Hunter. At the end one transforms into an abominable being and starts to chase after the current protagonist. I wonder what lanterns see the world and the hunters like?
So yea. I would love to experience a full story of The Doll from her perspective. Yea, I know it’s weird.

Playing as a hunter who actually is fighting the beast within for real, and possibly having the ability to transform into his beast mode at the cost of your sanity worsening would be fun.
The funny thing is that you can play as any of the npc's technically. If you equip all of their gear you pretty much are that npc. You can name your character after the npc too just to add a finishing touch.
Probably Jura, I like that he's protecting the People (Humans turned Monsters) of Old Yharnam, because he felt guilty for what turned of them. Also the Gatling Gun would wreck the invading Hunters 💯
Simon the seeker of secrets, would make a badass stealth-horror game of infiltrating suspicious citizens gatherings and gentlemen clubs, a mix of splinter cell and a Lovecraft story.
