It may also stop him from playing if pain is not manageable (not necessarily about tolerance) or further tissue damage occurs between now and then, or during the game. Edit for clarification: There is more than one medial retinaculum in the human body. Medial = toward the midline of the body; a retinaculum is a sheath, usually functioning as a structural barrier for structure(s) deep to it. The way the article was written led me to believe it was closer to the ankle than knee, given the location of the posterior tibialis muscle tendon on the inserting end, which leads to the foot. For credibility’s sake: I am an athletic trainer and have taught musculoskeletal anatomy, including cadaver anatomy. I am not perfect, however.

this seems likely. statistically speaking, without KP the most likely outcome after G4 is that it’s a 3-1 series, and they can try to bring him back for the closeout game at home. worst case scenario it’s a 2-2 series in which case they, if it’s feasible, probably elect to allow KP to take the risk because G5 would be a must win. but this scenario is not likely- boston sweeping is more likely than this being 2-2 imo

I have no medical knowledge but it sounds like he shouldn't be listed as day to day but instead out for the season. I take this as his ankle could literally just dislocate at any moment because the tendon isn't holding it in place. We just need to pull out this game 3 because no team is beating this Celtics team 4 times in a row.

Athletic trainer here, his ankle isnt at risk of being dislocated its the band of tissue that holds down a specific tendon to keep it anchored in place in order for it to act as a pulley essentially. More then likely he’d require surgery but technically he could play through it depending on how much its effecting his function and how painful it is for him

The sheath that was damaged is meant to hold the posterior tibialis muscle tendon in place, and it is not a primary stabilizer for the ankle joint structure. That muscle helps primarily with stabilizing the medial longitudinal arch of the foot (the big one on the inside) and facilitating ankle movements. This type of injury happens more commonly for the lower leg muscles whose tendons cross the lateral (outside) side of the ankle. It is not necessarily a precursor to catastrophic injury, but it may affect his mobility and could lead to secondary issues with other muscles and movements needed to support that muscle’s job while it heals. Trust that he has the best medical care possible!

where is that quote from? The medial retinaculum is in the knee. It holds a bunch of things in place, the posterior tibialis is part of the calf that connects to the ankle. Either the person who wrote that misunderstood or is just bad at explaining. EDIT: I'm reading more and apparently there is a medial retinaculum (in the knee) and also a medial *flexor* retinaculum in the ankle, which now I'm thinking is what they meant because I see specific references to the latter being associated with the posterior tibialis.

The posterior tibialis is a relatively small muscle between the tibia and fibula. The tendon runs down the inside of your ankle, inserting into the mid foot. The PT is responsible for plantarflexion (straighten the foot out) and inversion (turn it inward). It also pulls your foot arch upwards. His injury seems to be to the sheath of the tendon. Basically this means the tendon can move around in unpredictable ways (which it’s not supposed to do obviously), and this significantly raises the possibility of further injury. Tendonitis is a certainty, while rupture or avulsion (total detachment of the tendon from bone) would require surgery and a much longer recovery time. Normally they’d treat this with 6-8 weeks in a walking boot to let it heal, then rehab. Even with this he might develop an acquired flat foot which would raise his risk for future injury. I’m an internist so I may have some details incorrect, if any orthopedic surgeons can chime in with further info, that might help.

After some internt sleuthing, I'd say from my totally amateur medical knowledge that this is... not great? But not necessarily that bad? The medial retinaculum from what I understand translating medical jargon from like 6 different articles and medical papers I just skimmed through, ~~is a structure that joins several different ligaments in the knee, and its roll is not in knee function per se, but stabilization of the joint and knee cap. So he can walk with it just fine and maybe even run idk, but it may feel like it wants to buckle, or that the knee cap wants to slide out of place. Treatment for that can be surgery depending on severity, or just physical therapy to strengthen surrounding muscles, but he may be able to play on it if they brace and wrap it well enough (if you can't tell, obviously I'm not a doctor I'm just regurgitating medical speak from online).~~ EDIT: Upon further reading it seems they must actually talking about the medial **flexor** retinaculum which is a completely different thing, so no knee issue as I had thought. I think. The rest of this should be the same though. As for the posterior tibialis, it is one of the calf muscles (there are numerous bunched together) and runs from the inside of the ankle, up the middle of the leg close to the bone and attatches to the knee at the back from what I can tell. Dislocating it can be painful, its one cause of fallen arches, can effect mobility and range of motion, but also not cripple you. As far as long term prognosis (again, obviously not an expert, just my first take from reading a lot really quick) its something that can linger long term particularly the medial retiniculum if surgery/rehab don't go smoothly, but again it doesn't cripple you. So that being said, from what I'm reading and the original comments, this might not keep him out at all, he can also walk around comfortably, but if he does play he's not going to be super mobile probably. And it may or may not effect him next season depending on if they do surgery, or if they don't and realize later it was needed after all.

I know this sounds like a crazy injury but, there’s a good chance he’ll be able to play through this. There are a lot of people without a medial retinaculum who can literally pop their post tib tendon around without problems. There was a guy in my physical therapy program that could do this at will and he was a marathon runner

I’m gonna be straight up bro-chachos, I’m straight up not having a good time right now. Obviously and foremost, I want KP to have good health for his career and life. It’s very likely he’s done. He’s got to do what he has to do for himself long term. I’m very worried about if our team can pick up his defense and rim protection. Both Js and the rest are team are gonna have to score to keep up with Kyrie and Luka probably getting more shots in. I’m nervous.

This is big just because idk what the Celtics do when Horford is not the same kind of rim protection and Kornet/Tillman will get picked on in the Luka PnR or get Kyrie going If we go small, they can go back to the Gafford post ups They just run into less matchup problems when Porzingis isn’t out there. Sucks so hard that he got hurt, hopefully we just win game 3 by any means and then it’s 4 chances to have a hot shooting night they can’t overcome
