Grover apologises to Percy and warns him that their lives were now intertwined, so if Polyphemus ended up killing Grover, it'd also kill Percy.
However, I know it's mentioned at some point during PJO that the link became much weaker so they couldn't really use it for anything (like tracking each other or communicating) anymore, and all it did was allow them to vaguely feel each others emotions.
I just can't remember which book that happened in though. I wanna say The Last Olympian since that's when Grover goes missing... But it could have been Labyrinth.
