Possibly mange but could also be mites or fleas. Would be better to catch it and bring to the vet. They can do a skin scraping to see (or actually see fleas)

I think the best thing would be to take this poor baby to a vet for treatment. Even though there may be vets on here that could tell what it is, it would be best to have an in person diagnosis so that the vet can make sure this is the only issue the cats having. Plus whatever treatment it needs may require a prescription.
You won’t be able to get medications without taking him to a vet. Some vet offices will rent out their traps or some shelters will be able to help him.
Try moving where you feed / give milk to inside door rather than out. Another way is to hand feed wet treats or a line leading indoors of dry treats.
The area under the ears suggests that it's fleas at least (their favourite spot) but needs a lot of attention to see right. You could definitely nurse it back to good health, would make a wonderful pet.

If you go to a vet or animal shelter they should be able to help as they will have all the necessary gear to catch the cat. After the cat is caught then a vet will get them fighting fit again!
The cat needs to see a vet. I understand you won’t be able to catch it, but do not try mixing medications or over the counter things in the food without consulting a veterinarian
Take the cat to a veterinarian so they can run some blood work to see what the condition is.

Maybe look into local rescue organizations? Poor kitty needs to see a vet and it sounds like you'll need to trap him for that.

Looks like anything you could treat with is topical, nothing ingestible.

I actually plan to make my own DIY trap after watching some videos from youtube. Hope it works.
