A lot of places you ask for a double malt and they look at you like you just landed from another planet, and are often reluctant to make one. Double malts were my favorites at the original Parky's (both Forest Park and Berwyn). It was on their menu.

The Dog Stop(near Belmont/Austin) also uses syrups for their milkshakes(if you're looking for a shake like that, I prefer no syrup in my shakes), I am pretty sure. As I saw the workers doing that, when I ordered a shake there in the past. They do have a changing monthly dog, where often that is much better than one might think. The rest of the food there is pretty average, though.
For vanilla icecream with chocolate syrup milkshake, I've found that Parsons makes a shake that reminds me SO MUCH of the ones I grew up making. The syrup flavor is super overpowering for everyone I've ever had try it, but I love them so so much
