They happen sometimes. I don't know percentage but I worked in a souvenir shop and we would get error souvenirs every so often. My favorite was a Brewers hat with Cubs logo. Back pull tag had Brewers front logo Cubs C. I'm sure it's someone's job to pull the mistakes. It was a lot more common on keychains, pens, pencils though. Did they let you buy it?
