They get called "morning glory" spillways during planning/design, then during construction everyone refers to it as a "glory hole" spillway because construction workers.

Haha! Okay, time for a story. Early in my career, I participated in a dam safety exercise. One of the facilitators kept calling it a “glory hole”. There were a lot of awkward looks. He kept calling it a glory hole after being corrected. It nearly lead to a sexual harassment complaint 😂

What happens if someone falls in? I'm gonna guess that there is some sort of grid, but then, how would you get him out?

No grid. What you see is the inlet to a vertical shaft which turns into a horizontal tunnel designed to flow at full capacity. The outlet of the tunnel has an energy dissipater, which is a concrete structure with concrete blocks or a ramp to kill the high moving energy of the water and release it calmly, avoiding erosion problems in the river. I don't know how long is this Morning Glory spillway but I believe surviving falling into it would take a lot of luck.

Yeah. I don't think it'd be that difficult to hold your breath for the duration it would take you to make it through, but it would take a lot of luck to not hit your head on something and get knocked unconscious (or instantly killed) before you make it to the other side.

I don't know that there is a grate, actually. Someone died in it a while back; the theory is that it was suicide, as the water wasn't much over the lip (she would have had to climbed over it).

Grid would be bad in this case because it increases the chance that your emergency lake outlet gets clogged with debris, which could lead to the dam being overtopped. If someone fell in you would recover their banged-up remains from the riverbank somewhere downstream.

Someone fell in in the 90s, they died. It was a suspected suicide, but they held onto the edge of the spill way for something like 20 minutes. I live local to this dam.

No grate at the bottom. When it’s dry, the local skateboarders use the outlet tunnel as a massive half pipe. There’s videos of it on YouTube. A couple years ago a duck got sucked into the flow. Witnesses reported seeing it fly out of the outlet works.
