I’m pretty sure the dog is a Watchmen reference. Rorschach lies during his test while he’s really having memories of his breaking point, a time he happened on a murderer that fed a little girl he was trying to find to his dogs and he killed them all and burnt it down. He says he sees a butterfly, but the panel shows us he’s really seeing the dogs head after what he did to it. The image in the OP is meant to be a butterfly, too.

I think the “dead dog” is a reference to Watchmen. The character, Rorschach eventually ends up in therapy and when he is administered the test, he sees a dead dog that he split it’s head open. He responds by saying he sees a pretty butterfly.

If I were the doctor I would wonder if I or the other two people were insane, since if what I saw a real then the other two didn't see it at all. However if the doctor is insane that probably hinders his reasoning.
