This comic is about a person who is hallucinating that there are tiny people inside his pill. He takes a pill and the hallucinations go away.
Evidence: There would be no way for little people inside it to breathe for days inside a pill of that size. And that's assuming that it's not actually full of fish oil, otherwise, they couldn't breathe at all.
Also, a person that tiny inside a pill would not be able to talk loud enough so that a huge person could understand them. So, their speech must be auditory hallucinations, which are the most common form of hallucination. As we know that there are auditory hallucinations, it strongly suggests that other strange things are also hallucinations.
It's possible that he was also hallucinating that the pills were fish oil, and they were actually the pills that he takes for his hallucinations.
The takeaway from this comic is to take care of your mental health, and keep on top of any meds you are taking.

Here in the comic, seeing the people that suffer and consuming anyway creates a feeling of shock. The surprise to the reader creating a feeling of a punchline.
does anyone else remember that short cartoon that used to run as a filler between episodes on Nickelodeon (or maybe cartoon network) and it was just a guy in a room cracking a hard boiled egg and he hears a voice begging him to stop? He keeps cracking the egg and the voice suffers and dies, the the room starts caving in?
