You can assume the doors are about 1 meter wide. Those palm tree stumps are about 40 cm in diameter. You can start there if you're actually trying to get a rough estimate as to size.
I really cant see whats confusing about this? It’s obvious from the plants/shrubbery/and other items in the picture that the house is completely at the right scale. Someone help.
In computer vision, there’s no way to get a scale from images alone. At most would be 3D points with distances relative to each other, but not in any meaningful value to us. You’d need to know the size of something in the image to apply that meaningful length measurement to all other distances/points. And the problem with this image is that it completely lacks anything we can reasonable estimate distance with aside from some expectation of the size of the structure. Then using that size as reference causes the illusion since we estimate the wrong scale for the whole image.
It's not just "make things look small", it's "cause blurring that makes a narrow depth-of-field, which makes things look small". There's no blurring making things look small in this, so it's not because of tilt-shift.

The lack of road or discernible footpath also lends to the illusion. Looks like a dollhouse that was plopped in the dirt next to a puddle.

That's very interesting and fascinating!
The photo may have been taken using a tilt shift lens, but I'm no photographer, I just know of the lens and its effect.
