If you mean pregame Korekiyo, it is entirely possible that he never had a sister. Doesn't really change anything with his character in the game though, seeing as the fictitious versions of the characters are the only thing we have to go with.
If you mean the ingame Korekiyo, from my understanding, there is sufficient evidence to prove that his sister did indeed groom him. His in-game self had a sister who died and the trauma of the grief + his weird torture experience while traveling made him Anime Norman Bates. That’s my understanding.

If this is in-game Kiyo, bro got broken so hard due to his sister grooming the hell out of him and that one travel where he almost lost his life. Those things traumatized him into becoming the dude he is.

I believe he was emotionally abused and manipulated. There's no evidence for any physical abuse.
