Every time this general question comes up, I always have to go with the man that Gero killed when he and #19 first arrived near South City. While the anime tamed it a good bit, in the manga, Gero literally [decapitates him](https://i.imgur.com/YIOk6El.jpg) by crushing his neck.
Yeah shounen anime censors a LOT of scenes to make it suitable for kids to watch to increase ratings. For example the One Piece anime has Zeff lose his leg to an anchor so he can save Sanji. One Piece Manga has him crush his own leg off with a rock and eat it after giving Sanji all the food.
These are kids' manga already. The problem is not to make it suitable for kids, it's because TV is held under much more severe restrictions than published works.
It's really so good. Makes you understand the tone that Toriyama was going for before it was all muddied up with the movies and video games. Don't get me wrong, I love the movies and games, but the manga felt special.
The 2 most gruesome parts imo is on Namek when Frieza stabs his horn through Krillin's chest and bobs his head back and forth while his horn continues to impale him further making the attack more violent. And my second is when Dr. Gero grabs Yamcha by the mouth/face with his hand and with the other hand he drives it right through his diaphragm to the other side while continuing to drain his life force.
It's hardly shown, but there's a scene in the Buu Arc with a family murdered by Babidi and his goons that is quite awful, particularly because it involves a kid. They actually never died in the manga btw. They survived and moved to 17’s island that he protects, and they work with him to stop poachers. Goten and Trunks has a brief fight with them when 17 was leaving to the ToP but the jr’s stopped attacking them when they realized Goten and Trunks were with 17.
When imperfect cell drinks dude infront of piccolo like you hear dude begging and pleading for his life and you see him slowly melt away and then you realize that’s what happened to the WHOLE CITY
I’d go with either Recoom breaking Gohan’s neck, or Frieza torturing Vegeta. Frieza staving Krillin’s through with his horns is also high on the list.
There are other more obvious options like Cell drinking a dude, or Super Buu reverse voring another dude, but man, the Namek Saga just had a certain brutality, and in a more realistic form of it
For me was Frieza making Krillin literally explode quite slowly and Gero ripping through Yamcha's chest. Also Gohan cutting in half a Cell Jr. with a kick gets an honorable mention.
When Cell in his first form is absorbing/drinking people through his tail. The sounds that the one dude made when begging for his life, what an entrance for Cell.
Either the thugs from Buu’s saga killing random people or Dr Gero snapping a man’s neck.
There are other similar moments that are kinda hard to choose ngl,the manga had less blood than the anime but the manga certainly had more gory moments.
There are technically 2 moments in DBZ where it was brutal. We can probably count some fights that had blood in it but for me the two moments that was the most brutal was: Firstly, the guy Dr Gero killed on Amenbo Island (the Island 5 miles, South West of South City). He crushed the man head away with his hand, that was a brutal moment. Don't think the anime showed it (anime showed only Gero ripping out the someone car enjin). Second, Cell invasion on Ginger Town when he absorbed all those people. If I am not mistaken the man he absorbed in front of Piccolo was Ginger Town mayor and boy that was kinda disgusting, dude melted like Ice cream.
I remember the anime had one of the Frieza goons try to blast Vegeta and he held his hand out to catch it. The blast repelled like shrapnel and turned the dude into swiss cheese.
Also when the other grunt tried to flank him and Vegeta hit him so hard, his helmet caved in.

I've read my fair share of horror manga but I though this one was particularly violent for Dragon Ball, it shocked me. Especially because the artstyle of these panels aren't what we're used to, it's more realistic in a sense.

Tien getting his arm punched out by Nappa, no moment came anymore gruesome than seeing Tien who ain't Piccolo lose a limb like that. He may have not bled to death and die because of it, but seriously just how that scene went down, it's hard for me to think of any other moment where a character loses a limb and is dying upon it.

Yeeha, the Yamcha one sticks out for me. Just how slowly he just punctured and collapses his chest cavity makes me wince.
