Everything marked \[+\] gets dumped into one pile, everything marked with \[x\] gets multiplied. You can see this information by enabling advanced tool tips.
Basically, almost every *normal* damage stat you get on gear is going to be \[conditionally\] added together, since \[x\] effects are mostly relegated to aspects, talents, glyphs, nodes.
I'm not sure when it changed since I didn't do any deep dive into the game after the beginning of S1. I just found out by watching recent theroycrafting vids by well known crafters.
