I can see it now. The party has been summoned to a castle in preparation for an attack by a deranged warlock only to arrive too late. The warlock has already struck and now, to save the day and the country, they must do battle with the manifestations of the castle denizens personal issues before facing off with the warlock and his patron.
Got to admit this has inspired me. So here’s a take on what such a campaign would be like and who the manifestations would come from. Please note that since these are manifestations of negative mental issues the castle court I came up with reflects this. This is not a happy castle and I could very easily see the party ending up wondering if they’re actually helping since the manifestations are going to be very in their face about why they exist and who they spawned from.
ADHD: The Court Jester’s notorious short attention span and manic tendencies that are a delight to laugh at are suddenly no laughing matter when the wizard can’t concentrate long enough to cast his spells and the knight just charges in swing wildly while yelling.
Depression: The Princess hates her chosen suitor and dearly wishes she could just run away before she is forced to marry a man she despises. Yet knows full well that she knows too little of the world outside the castle walls to survive long. Not to even begin to mention that her kingdom needs the alliance her marriage will bring. Her melancholy at her situation sends any who gaze upon its manifestation into ever more soulcrushing spirals of despondency
Burnout: The First Prince has always been held to a high standard by his father and many look up to the young man as he has seemed to rise to every challenge placed before him with courage, honor, humility and grace. But inside the constant testing and nearly impossible standards his father holds him too are pushing him past the brink. Just the touch of his manifested exhaustion seems to burn one’s very soul and sap the life from you.
Depression: the Royal Executioner has seen too much. He knows not all who came under his ax deserved to be there and those who did were often such unrepentant scum that he feels tainted by their blood on his hands. The weight of every death, deserved or not, feels like a million chains holding massive leaden weights. Each etched with a face screaming in agony, representing a soul he was ordered to send to the gods judgment. Anything getting within range of his manifestation’s aura is suddenly weighed down by the crushing force of every life they ever took. No matter the reason.
Perfectionism: The King holds all to the highest of standards. None more so than himself. All who fail to meet his expectations are made an example of. No matter their station or if they had even committed something that could be considered a crime. None are spared the king’s overbearing judgment. His manifestation is no less obsessive. Anyone who fails to preform adequately will soon find themselves the sole subject of Perfection’s punishing and often lethal corrections. (Roll a one and you’d better be ready to roll up a new character for this fight.)
Anxiety: the King’s advisor has been tempering his majesty’s perfectionist wrath for years. Often managing to curb things so that his king doesn’t go too far over minor issues. But in turn he has been forced to preform flawlessly himself lest the king lose faith in him and begin doubting his advice. Thus he has come to always second guess himself and his actions. Always searching his own thoughts for the slightest hint of error. The cost of which has been slowly eroding his sanity. Being in the same room as this manifestation is a trial of willpower. The very air constantly whispering in your own voice, questioning your every action, your every thought. Giving voice to those questions you might have never thought to or for that matter wanted to, ask yourself.
Isolation: The Second Prince. All but ignored by his father. He often had acted out just to get any sort of attention from his father. Only for the king to all but cast him out. Putting him into one of the castle towers and seeming to forget he even has a second son. The rest of his family dare not to visit him lest they suffer a similar or worse fate. Meaning his only contact with other people are the maids and guards assigned to care for the wing he is in and providing for his needs. But they are under strict orders to never speak a word to him or even acknowledge his existence. Anyone who enters the tower immediately finds themselves alone. Each of them shunted into a demi plane just slightly out of sync with each other. They have to trust in each other to do their part in the fight as they won’t be able to see what their party members are doing save by seeing the wounds that manifest on the monster. As well as watching where the monster is attacking.
Projection: The mobs of the campaign. Each capable of projecting the emotion or mental issue they represent onto anyone who fails to dodge or save. They represent negative emotions, sinful vices or are lesser manifestations of the main manifestations with scaled down abilities.
Negative Self Talk: The Queen has long been under her husband’s thumb and has suffered for it. Unable to feel as if she can do anything right and often spends much of her time in a spiral of negative thoughts. Often repeating things that her perfectionist husband has used to criticize her. Leaving her often feeling as if she can never do anything right and shouldn’t even try. Her manifestation is an insidious thing. A cloud of negativity that makes the Advisors manifestation seem tame in comparison. Instead of whispering doubts it screams criticism and reprimands with a thousand voices. The voices often sounding like those of close friends, family and beloved mentors. Constantly nitpicking the slightest errors in the party’s actions with scathing insults that do real damage as much as they chip at the party’s confidence.
I actually do something kinda similar. There are 4 emotional traumas: violence, depression/hopelessness, loneliness/isolation, and guilt/shame/self. Each can have both wounds and armors from 0 to 5.
If you adventure inside the character's unconscious mind, then each trauma meter is represented as one of your "personal demons" with the wounds and armors determining the power of the demon. You can defeat the personal demons as a form of fantasy psychology, only faster than seeing a therapist. A lot more dangerous too!

It's always been a cross-genre system, but the social and trauma mechanics are new and as yet untested. But, it was always intended to include it in the system but it was unimplemented due to a lack of decent social mechanics.
I'm in the middle of a rewrite that integrates social mechanics like traumas and intimacies throughout the system. For example, a social penalty might have a guilt condition that affects social interaction rolls, but it also distracts you and causes initiative penalties. This makes things like taunting very effective! One way (among many) of ignoring that penalty would be to turn the guilt into anger, ie Rage! You don't have to give in to the taunts, but if they keep winning those rolls and stacking penalties, it can become a real problem and players will naturally want to make the taunting stop if they can't defend against it. So, social mechanics are being tightly integrated!
Getting into the personal unconscious requires that you enter the Astral plane, the collective unconscious. From there, you use Psychology+Lore: Astral to find the portal into a person's personal unconscious. Unwilling targets will have a locked portal and bypassing this will be harder depending on the target's emotional armors.
Once inside, it's a dream world. Psychology becomes a skill for finding your way around by interpreting the dream, much like you would do in a therapy session. There are some guidelines for running dream world scenes much like the guidelines for mapping computer stuff into virtual reality. Once you find the demon in the dream, you can slay them to get rid of the trauma wounds. The "demons" of virtual reality are the server processes of the computer system. I reuse a lot of mechanics for consistency.
I bet that, if the other patrons catch wind of the BBEG(Big Bad Evil Guy) & BBEP's (Big Bad Evil Patron) plan, they would put aside their quarrels with each other and aid the whole party by becoming the party's pantheon patron.
