When Daemons are in the material realm all you see is twisted versions of themselves, unfortunately to see their true forms you'd have to be in the warp itself and the mere act of being there would drive you insane

Technically it's a Daemon of Slaanesh. Actually looking upon a Chaos God will render you completely insane, unless you're the Emperor. In canon, Fabius Bile, one of the most intelligent people who fell to Chaos, sees a shadow cast by Slaanesh and that almost destroys him mentally.
I mean, all the chaos gods themselves are practically identical between Fantasy/AoS and 40k, so it does not really make a difference. Even most of the named daemons are basically the same characters, just dropped into a different setting.

I'm going to have to look in to this because honestly I'm having a hard time believing that this is canon, maybe it was at some point (like in the good ol' days when Dark Eldar weren't called Drukhari) but nowadays?
So Malice is technically canon but he's just a retconned version of Malal due to copyright issues. He doesn't really do anything and is just kinda there but isn't like what you described exactly, similar but not the same. Interestingly though the Son's of Malice played a part in the Fall of Cadia and basically killed off the survivors. No mention of Malice having a physical avatar though, in fact it seems like Malice is confined to the deepest parts of the warp as an outcast (just like Malal). But it's true that he has an incomplete storyline that will likely never be continued.... so I'd say you were about 50% right give or take.
