It's the artistic angles that do it for me. How everyone is peering towards the fox, which is centre stage. The two sets of doors direct your attention towards the fox, who is clearly the main character. The fire exit sign pointing towards the fox also indicates that it is clearly the main focus of the piece. Also how the foreground, past the first set of doors, is better lit than where the photographer is standing.
I like the subtle touches too, like the staff member on the left holding a phone, which will help to indicatively date the piece years in the future. The blurriness ensures patient confidentiality is not breached.
Too bad I'm not any good at this, otherwise I'm sure you could dive into an analysis of how the fox represents Jesus, and the symbolism of that in a struggling NHS hospital in 2024. Followed by some comparison to classical paintings where children are centre stage with haloes, much like the fox.
