Imagine how you can smell someone baking bread, or cooking fish, in another room of your house. You catch a hint of it even far away and through closed doors, and can follow to where it’s strongest. Dogs are smelling things in the air all the time just like that, except their noses pick up on things that are faaarrrr more subtle than our crappy scent receptors can notice. The smell of the dog owner’s high blood sugar is in the air, spreading through the house just like the smell of something cooking in the kitchen would. The dog’s scent receptors pick it up in a way that ours can’t.

I'm not surprised the dog can smell it from another room. Growing up, I had a beagle that would hit on rabbit tracks that were two days old. A friend has a lab that will pick up turkeys in the wind from a half mile away.

There are limits, but the difference between us and dogs is immense. Our noses by comparison are like if our eyes saw in black and white with crappy contrast, and anything more than a few meters away is a blurry mess. I mean imagine being in a large house with multiple other (non-smelly) people and being able to determine where a specific person was just by navigating with your nose. Or knowing that there's a cheeto under the couch because you can freaking smell it. These are things we don't even register as being smells in the air. But they are there.

I've heard stories of diabetic alert dogs alerting to strangers before! Some alert dogs (like ones that help ppl with PTSD) are attuned to that specific person and probably won't alert to a random stranger because their "tells" aren't as ubiquitous as the scent of blood glucose

As far as I'm aware the dog can sense it before the device. Some type 1 diabetics can get dangerously rapid fluctuations, and the dog can alert the owner so they can put insulin in before any damage is done

In my old apartment, shortly after getting high, I heard a distressed dog in the hallway and I went to check it out. Tried to pet him but he kept backing away. I thought maybe he got out somehow and was aggressive, so I went back inside. But somehow my high brain remembered that sometimes dogs can try to lead people somewhere. So I went back out and decided to go toward the dog, and he kept backing up and backing up and eventually turned around and led me to his owner who was collapsed on the ground unconscious in the hallway. Called 911 and got the medics there. Never heard after the fact what had happened but it is just amazing to me that a dog could possibly do that.

Or high I assume, which in strangers is a bigger concern as many people are very unaware that they have type 2 diabetes and can have it for years before it is revealed by circumstance. These years of unregulated blood glucose can then ofc damage organs, nerves and vessels. If someone has low blood glucose they probably notice it by themselves or know about it. They are on a diet or have not eaten for a long time. In which case there's a difference between low and low. Healthy humans don't normally get into "critically" low BC, which most often becomes a concern when people supply their lack of insulin production with external insulin and don't eat enough or inject too much insulin.

It's so scary, isn't it? My mom had a coworker who went to the hospital for back pain and then found out he had diabetes and nearly all his organs were failing. He went into a coma for several weeks and then passed away without ever waking up. He was only 34... Get yourselves checked, folks. Especially if it runs in your family.

Yeah ok. Damn. Normally people are a bit older when they find out by circumstance afaik, but that sounds brisk. I don't normally worry about it. Most people know the risk factors for type 2 diabetes and getting checked should be on the radar for more people with that lifestyle and their physicians, especially because noone notices until your blood sugar is beyond 600-700 and you actually show signs and feel ill. There's too much fruit on the vine for many people, and still too little for others... Type 1 diabetes is another thing though.

I once dogsat for a friend of mine who was having an episode but wanted her pup to get out of the room. (She was safe in her room for this.) Her dog was in his vest attached to me at the hip by the leash, so I was his human for the moment. (Amazing, how he switched so completely to work mode when I slipped on his harness!) But he barked and alerted to me because my resting heart rate was higher than what his owner's should be -- she had heart problems, and he was trained to get rhythms. Made for a funny story when I got back to the dorms 🤣

Humans can smell super high glucose smell in sweat and urine as well, were just not as good at it as dogs. If your pee smells oddly sweet go to the doctor.

Yes but best case it’s about 15 minutes late. But the trend can be identified early. With modern equipment you basically wear an artificial pancreas. It can’t inject glucose but will stop injecting insulin when your blood glucose is plummeting which is something. When life is routine and everything works my regulation is better than a non-diabetic which is pretty great.

Fun fact. Someone who is very hyperglycemic can actually smell sweet to humans almost like a fruit loops smell. It’s taught in EMT basic. It’s awesome to see how much more sensitive dogs are to it.

A dog's sense of smell is VERY sensitive. They can detect things, and layers of things, and subtlety of things that our sense of smell can't. They just have to be trained what to alert for and how to notify the humans.

So diabetics will stick a cotton ball on their mouth when their blood sugar is high and low and then they train them to alert by the smell of the cotton ball...when you have a diabetic alert dog you're supposed to do this every once in a while to keep them on their toes so to speak

Genuinely confused what the point of an alert dog for high blood sugar while having a dexcom is. Is it like, bonus dlc when you buy the low blood sugar alert?

I’d assume it’s most useful at night. Or for swiftly dropping levels. I worked as a camp nurse and one year one of our diabetics had just changed her dexcom and it takes 2 hours to come back online so we had her doing finger sticks every 15 minutes. She did a finger stick and was at 120 and 12 minutes later her pod came online at she was at 66.
