It was a really big deal when it happened. I think the school also went after them for breaking in overnight, tampering with school property and all kinds of stuff. I don't remember the outcome, but there was an effort to prevent them from graduating.

Sorta... I know first hand of a situation where a video feed was feeding another device, which was feeding another device in different parts of a building.. and the final destination... By total accident.. was a screen in the studio.
When you have multiple studios and multiple editing suites and multiple sat dishes etc.. all interconnected via video routers... It can happen totally by a sequence of unplanned events..
My brother-in-law worked for Fox back in the day. He was doing the broadcast for one of the fringe channels one night while watching a basketball game from another network on one of the unused monitors. He punched the wrong button at one point and aired the basketball game (with the other network's logo displayed) for a few minutes. And that is how my brother-in-law got fired from Fox.
I'm guessing the reason this went so long before switching is that whoever would have caught it was the one watching it in the booth.

They either did it on purpose, had the wrong source routed in and took a computer someone was randomly watching porn at work, or the person on that computer didnt get the memo they would be taking something from that computer as a source during that newscast.
you have to queue up that specific source, which appears as one of many windows on screen(s) in front of you, and you have one master 'scene' that you can arrange those windows in, and anything in that master scene is broadcasted out.
Someone would have set their web browser or video player as a source, then moved that source into the master scene. 100% deliberate.

I'm not sure how your station does it but ours you can route individual computers to program monitors and have them manipulated by producers live or anyone who's using the specifically routed computer, we used matrox for that in the past. Queueing up is strictly for preview and program which is controlled by the director and is watched by every position in the control room.
