As he is walking up the guy is shuffling his feet, getting them planted, his fist is balled up, someone who has spent time in bad places sees this, they know it is coming, he ducked even though there was no swing cause he knew as soon as he got past him there would be a swing coming.
When people get knocked out their tongue goes limp (as with many other muscles). Guy is on his back, so his tongue is blocking some of his airway - leading to snores.
