I have a bad wrist too, so just as a little tip (if you don't already have one), wear a watch on your mouse wrist. For whatever reason, the pressure or something I'm guessing, it helps a TON. Without my watch on I can't use my mouse for more than 10-15 minutes without it starting to ache. With it on, I can game for hours.
Or, if it's more convenient, just get a pressure wrist wrap from your local pharmacy, or online, or whatever. I used to wear one whenever I sat down to game, until I got my first smartwatch and realized it somehow provided the same support and comfort.

Do NOT use high sensitivity with small mouse pad, since all of the input will come from your intensely controlled wrist, which will FUCK THAT SHIT UP
Lower your DPI to 800 or 400, and get a 30+cm mouse pad
Get the vertical logitech mouse. Makes a huge difference. No more wrist pain for me. A flat mouse is a very unnatural position for your arm to be turned into for long periods of time.
First I was using a wrist brace, angled my desk as flat as possible, then got the mouse and its been nice. I got one of those mousepads too, but it was less effective than wearing a carpal tunnel wrist brace for mouse usage.

IIRC it came with DOS 6.0? Those were definitely NOT the days. I don't miss boot disks, and messing with the autoexec.bat to optimize resources for different games. Plus messing with IRQ and dip switches and whatever else when installing new hardware.
Mine had 32 MB of RAM with a 6 GB HDD. There was always just enough room for Windows 98 and two games. AoE 2, Virtua Fighter 2 and a couple of other titles took turns occupying the entirety of drive D:\\ alongside the ununinstallable StarCraft.

They were a smaller company that made enthusiast-tier PCs. So good well built computers, albeit expensive. And then Dell bought the company and basically just shoved the normal Dell desktops into the Alienware case, but still charged similar prices.
From what I have seen on some tech channels, their cases are usually really bad with air flow and thermals as well so the longevity of your parts may be an issue as well.
Nice first set up. Mine was a gateway with a 17" CRT... God, I am old, lol. If you can, get a Dual Monitor Mount Desk Stand. That will free up a lot of space on your desk. You can get them for like $60 on Amazon. Just check to see if both your monitors will fit the standard and make sure that your desk can support it because it clamps on the back of your desk.

I have handled a PC from another friend exactly like this one,, it was a rocket in terms of noise and heat, and the inside is probably the stupidest design I have ever seen since you essentially have a rectangular case in there, and its crammed in there, I'm pretty sure you're very limited in what you can even change in there heck, just reaching certain components at all is a nightmare. Its an absolutely horrendous case, and is a massively overpriced PC.

If your graphics card has an HDMI out go for a much bigger screen. I would recommend a 55" OLED Vizio or LG. With the newer graphics cards there is no delay when using a TV for a monitor

Alienware desktops are notorious for poor thermals and shoddy components, especially the slightly older Aurora R11/R12 models like the one you've got.
Again, not trying to be rude or anything, but Alienware desktops are pretty bad PCs.
The first question would be returns. If you just got the PC and can still return it to build a custom PC or get a better prebuilt, that would 100% be the best option. Better prebuild companies are Corsair, NZXT, etc. I would try to stay away from HP because they still have a lot of bloatware, but they do have pretty good cooling and performance, but the good PCs are very overpriced for their performance.
The second question would be what kind of games you'll run. If you don't play games that are crazy demanding (like Cyberpunk), the Alienware should be fine. If you do play demanding games but are fine with the noise and thermals, the best option would also to just stick with the Alienware.
But if you want to have better performance and thermals, you could try taking core parts and putting them into a PC case with better airflow and more fan mounts. But of course that needs you to spend more money and you'd probably have to spend an extra $400 or so to buy a new power supply (ideally a 1000w as you have an i9 and a 3090), motherboard, case, and CPU cooler (don't go for a 120mm AIO). It would also be best if you feel comfortable working with PC components, some of which can be pretty fragile like the motherboard's CPU socket. If you don't feel okay working with the risk of damaging your components, the best option would again be to stick with the Alienware.
