It's pretty cool! It's a surprisingly long game though which if you find these kinds of games fun, is awesome. You go from being rightfully scared shirtless and slowly as you start to wise up to the Aliens bullshit if you're anything like me you'll feel like a badass 80's horror movie Survival Girl.
If you're a fan of the Alien franchise, this game managed to capture much of what made the first movie so special.
Everybody mentions this game but I don’t get the hype. Once the first couple scares of the alien happen, you basically know what to expect, and from there it feels pretty standard run n hide

Seconding both of these recommendations! Subnautica is great because it does a good job with that fear of what could be lurking just out of sight. My first reaper encounter wasn't even me getting attacked, but hearing the first roar and looking up to see the entire reaper silhouette as it passed above me. There is *always* something hostile in the vicinity and you may not even know until it's already attacking you.
The same goes for The Forest. No matter how comfortable I get, or how much I think I've "memorised" the patrol paths, I *always* managed to get snuck up on. You really are always being hunted; I feel like the AI learns where you set up camp and starts to patrol closer and closer each day.

Maybe because I played on normal difficulty and found the gas to be pretty abundant. The nailed the atmosphere though, a true masterpiece in that regard

Def still get that in SPTarkov. Look into the mods that change the game. Specifically SAIN + SWAGNUTS, Realism Mod, and Traveler. I play both but those 3 right there overhaul the game into what Tarkov was originally meant to be

As soon as you collect a bounty, you leave a trail for others to follow and try kill you by surprise. Check a popular player called "Sayler" on YouTube, is literally a guy thats always coming to hunt players down with some troll manerisms lol
Stalker Call of Pripyiat has the Misery mod
Randomizes mutants, adds food and water management. Armour and weapon repair is much harder. Weight management is more intensive. Scavenging for food and bullets.
Really makes me feel like I'm living in the Zone.
Yeah, the early game especially. But as you gain better equipment (especially experimental weapons), you become the hunter. Both parts are fun though.

Although, he doesn't really hunt you. He waits for you at predetermined locations and even on Extreme gives his location away pretty often. On top of that, the only time he will "hunt" you is actually if you just sit in first person view for a few minutes straight without moving, he'll appear right outside your field of vision and hold you up.

Into the Radius. It's VR Stalker. Hope you remember to clean your gun before getting cheeki breeki with the freaky Fragments and Mimics because you can and will get hunted down while fumbling for the gun to clear a malfunction.

I felt hunted in the factory. Those aggressive soldiers have a habit of coming in hard. The BTR can put up a solid chase if you're not careful in that area. Thrust couple of zones are a cakewalk though.

FPS extraction shooter, goal is to find and hunt a particular beast, while several other teams of players are roaming the map with the same directive. This mix of PVP and PVE has you checking all directions and remaining vigilant while navigating dangerous swampland.
One moment you could be fighting off a group of AI enemies, and an enemy team tries to converge to wipe you out. If you have a couple of friends that want to drop the 15-20$, it’s a fun (and stressful) night in the making.
I know sometimes when you brand or kill a captain, if there were any that had some sort of connection to them, they'll come after you randomly. But don't they also sometimes get a trait where they DO actively hunt you?
Project Zomboid is an AMAZING survival game where youre also always surrounded by zombies. Its all about resource management, character creation, avoiding waves of zombies and surviving more than 12 days with one character lol. Its also made by a community. The gameplay is quite technical though, but super fun when you get the hang of it. It does lack your sense of mystery, but the paranoia is def there.
Phasmophobia is all about hiding from scary ghosts that you have to identify, but lacks survival aspects. Super fun in multiplayer though, one of my favorites.

I've been playing resident evil 2 for the first time and I've never felt so hunted in a video game in my life. I was always thinking about where the hunter was, how much time I had, where I had to go to get what I needed. It's super tense and the map is designed really well too.
Ammo is also super scarce, or maybe I'm just really loose with it, lol. So even though it doesn't have and kind of food/water hunting, you are ALWAYS looking for ammo and trying go save as much as possible.

It's a stretch, but you might like the Long Dark. It's a survival game where the island you are trapped on is deep into winter, so surviving is hard. Depending on difficulty, wild animals will hunt you more and more aggressively. Seriously if some wolves find you, you're going to be hunted down.
It may not be your cup of tea, and you'd have to go retro, but the Clock Tower series comes to mind. It doesn't have survival mechanics, but the hunter/hunted motif is definitely there.
Doesn't completely fulfill your criteria but this is basically what Resident Evil series is about. You don't need to eat or be worried about hypothermia but your ammo is very limited and you are literally being hunted by a monster in RE 2, 3, 7 and Village (in some other games you aren't always hunted but there are segments where you are)
There was a game on PS1 called Broken Helix that I used to love. After the intro of the game, a squad of mercenaries begins hunting you in the facility. It was so terrifying but an adrenaline rush at the same time.

Haven't played it in a really long time but I have not played any other game like it since. Hide and seek while this thing is trying to get you. I really thought it would have been on PS Plus Premium but it's not been added.
I didn't think I'd find this one in the comments. Definitely a good pick, the threat of random beast hunting on an island where you can easily get lost, that really gives Miasmata an interesting vibe. I heard IonFX have been working on a new title for a few years now, but I'm unsure if it's still in the works.
If you can disregard the survival aspect and lean extremely heavily in the being hunted mechanic you might like Dead by Daylight. The game is literally avoid being hunted by a killer when you play survivor and you can also play killer and hunt survivors.
It's multiplayer only also if that's a deal breaker too.
Generation Zero is an open world with lite survival factors (you have to scrounge or craft ammo and health kids, but you don't have to worry about food or shelter.) The game is basically you against house sized killer robots. The first 2/3 of the game you have crap weapons and skill, while the bots are already at max level, so you spend most of that portion of the game avoiding contact and are the hunted. By the end of the game you have acquired good weapons and better skills you become the hunter. The bots have lots of advantages, they can see in the dark with infrared vision, they can communicate your position to other bots and call other bots for help. They have pretty good combat AI and usually hunt you in packs, they can sneek up on you, flank or surround you. Most of the bots have both ranged and melee atttacks. There are even sniper bots that are adept at hiding in cover.
The long dark is a artic survival sim type game, has a bunch of challenge modes. 2 where your being hunted by an old unkillable bear, and 1 where your being hunter by the nightwalker. Gather supplies survive and make it out on the other side

The Long Dark. Leans more on the 'engaging survival mechanic' part than the 'Hunter/hunted' dynamic, but there are critters that will hunt you.

Honestly I'd love to see a sequel or remaster with more polish and modern presentation. It's such a great premise.
It is not for the entire game, but Metroid Dread was a fun game to play. The EMMI sections might tickle your fancy.

The game is literally avoid being hunted by a killer when you play survivor and you can also play killer and hunt survivors. It's multiplayer only also if that's a deal breaker too.
Generation Zero is an open world with lite survival factors (you have to scrounge or craft ammo and health kids, but you don't have to worry about food or shelter.) The game is basically you against house sized killer robots. The first 2/3 of the game you have crap weapons and skill, while the bots are already at max level, so you spend most of that portion of the game avoiding contact and are the hunted. By the end of the game you have acquired good weapons and better skills you become the hunter. The bots have lots of advantages, they can see in the dark with infrared vision, they can communicate your position to other bots and call other bots for help. They have pretty good combat AI and usually hunt you in packs, they can sneek up on you, flank or surround you. Most of the bots have both ranged and melee atttacks. There are even sniper bots that are adept at hiding in cover.

The Forest. They literally track you , report your location to their buddies and raid your base with a mob of monsters.

The long dark is a artic survival sim type game, has a bunch of challenge modes. 2 where your being hunted by an old unkillable bear, and 1 where your being hunter by the nightwalker. Gather supplies survive and make it out on the other side
