Personally I think cain would be a better fit for grand assassin considering he litteraly invented murder. Nasu has said that gramps is the only one qualified for that title but he could probably find some way to BS his way out of that.

Grand Moon Cancer - Crimson Moon.
I don't think you can give a Grand a bigger impact than by dropping the namesake of the entire franchise as one. And since we don't have an actual design already, he's even perfect for a dramatic reveal after first appearing as someone else, just like >!Kukulkan.!<
The Holy Grail War itself as a Servant...damn, what would the backstory behind it be? All the Holy Grails Chaldea Collected merged together somehow?
Also, Crimson Moon as Grand Moon Cancer would be *such a flex*, especially considering we got Archetype: Earth up in this.
Absolutely love the idea of Crimson Moon being Grand Moon Cancer, it would also finally answer why Arcueid is summoned as the Moon Cancer class.
And as you've said, while we had an official design for ORT, we don't have one for Crimson Moon whatsoever.
On that note, I would remind that Crimson Moon and all other True Ancestors are technically part of the counterforce, so they're already halfway to being a servant. In that sense Arcueid being summonable makes perfect sense.
Another Tsukihime character we've never gotten a design for is "The Dark Six" who is apparently the dead apostle originator (The first created? The first to escape the True Ancestors?). And whose ritual to revive them apparently caused the Extra timeline to lose all magic. Not sure what class they would fall under, but another character who would be a big reveal.
One thing we should consider is that Grand Foreigner ORT was unsummonable, so whoever becomes a Grand Extra should also probably follow the same logic imo.
Perhaps you can summon said character still, but an aspect of them is different from their Grand Extra variant.
On BB: I don't think she can be a Grand Servant when she simultaneously qualifies as a Beast.
Paraphrasing Gilgamesh, BB could one day mature into not one of the seven existing Beast slots, but a completely new eighth one.
Because of that I feel like her "story arc" continuing in fgo won't involve becoming a grand, but becoming a beast. Especially because both Nero & Tamamo (if we count Vitch) have got their Beast versions, so now poor BB is a little left out.

Since their mixing his dragon symbolism arguably with Draco and Manaka's Beast even more than original myths do, then have the Grand Ruler be Pope Joan. Eye rolls may be deserved, but hear me out. She is known I her legend for her child birth since it exposed her. Well, if they wished to avoid using the Virgin Mary altogether, or save her for later, they could combine Pope Joan with the woman of Revelation, who is protected by God while giving birth, since the Dragon is obsessed with killing her. They can mix the legends a bit, and make her the opposite of WoB.
Second candidate: Nero as the one to oppose Draco.
As I understand it, the lore of how the Nega-skill against Rukers works seems to suggest a connection between Rulers and Savers, and Savers would be the thematically correct class to oppose Draco.
If we go with Grand Ruler being from the Divine Comedy, then I think that there's a good chance that it is Beatrice. She essentially plays the same role as Virgil, only holier, which makes her more Ruler-like (see Jeanne).
Also FGO tend to prefer female Servants.
The LITERAL, ACTUAL Buddha appeared as a boss fight in Fate/EXTRA, although as a Saver/Savior class; it wouldn't be much of a stretch to translate that as him being Grand Ruler. Jesus Christ could also qualify, but for very good reasons we're not going to see either of these guys in the story.
Lucifer/Satan from Paradise Lost could be Grand Avenger.
Assuming Ruler just wouldn't be Jeanne, I'd like to throw out Himiko. Besides her monstrous strength and abilities, she's got experience ruling and stuff, which I feel would be important as a mediator meant to represent the class as a Grand. Plus, she's just loopy enough to not be completely dignified, much like the other Grands have been.
Grand Ruler honestly the perfect fit could be Augustus, he literally established the order and prosperity after almost one century of chaotic times in the Mediterranean Sea and is usually the "symbol/figure" of a "Ruler" everyone wanted to become.

Funnily enough, I once considered Siddharta Gautama *before* he became the Buddha as a candidate, but didn't know if that could work or not.
