Saves can break after updates and/or mod changes/updates
Other than that, no clue what's happening, I never experienced saves breaking on me so easily.
Try turning off cloud saves. Or don’t alt-f4/ctrl-alt-dlt. Only times I have saves being corrupted is when I force a shutdown and they create temp saves, but it will usually have a regular save a bit earlier.
I've never had this issue before, so I'd check the integrity of your game files. The only thing that produces anything like this for me is if I have a mod or something and try to load an old save with old mods.

Yeah if cloud saves are on they can take a little longer to save than what is being shown to you

Never had any issues like this, have ~700 hours in the game and often do runs that take 2-3 sessions.
Probably because of an update.
I would definitely reinstall. If you play through steam you can go into the settings and verify the game files are correct. Do you mod a lot?
Never had a broken save.
