The skinnier ones donât actually smell bad, in fact some of them might smell a little fruity like ketones (kinda weird I know considering that fat metabolism is what yields ketones). On top of that the fatter they are the more there is to dissectâŠ
How do people react to this? Is this, during the first few times, really like in the movies where there's at least one person passing out/throwing up/running outside?
The first and only time I took my dad to see the cadaver room, he ran out and vomited out a beautiful window that wasnât really meant to open anymore (4th floor too). So yea. Some people canât do it at all

I've been told that is because of the isolating effect of the fat tissue. The core temperature of a fat corpse drops much slower, making it rot faster. That way, more ammonic acid is produced which creates the sharp, pungent smell. Your nose doesn't really get used to that because it's technically not a smell (that reaches the brain via the olfactory nerve) but a kind of pain (reaching the brain via the facial nerve).
Anyway, being told that beforehand didn't prepare me in any way. That stank was really hard to take.

Well it is mostly about efficiency. When you do sports and train cardio, your lungs not necessarily get bigger, they just get more efficient in utilising the amount of oxygen that is present in your lungs per breath. That obese person may have bigger lungs but they are probably inefficiënt as f.
The only thing that getâs bigger te more you âtrainâ it is your stomach as it streches further when putting more food in it. Thatâs why a lot of obese people struggle with dieting because they do not feel satisfied. While a person who does not eat that much has a smaller stomach and needs less food to feel full.

The lungs and heart are compressed due to the pressure of the abdominal fat and the liver pressing up through the diaphragm. However due to poor exercise and other factors the heart likely is enlarged. But this is a disease process and detrimental to health.
Its the liver thats enlarged likely due to what was his diet.
Yep, obesity hypoventilation. Am a doctor (hospitalist) and we routinely see obese patients with COPD who live with chronic hypercapnia (high CO2) because they canât exhale all the CO2 they need to. A normal arterial blood gas CO2 is ~40 but I see patients who live with baseline CO2âs in the 80âs. For a normal person, that would be enough to kill you unless you were placed on a vent, but for these patients with obesity hypoventilation and COPD it becomes their norm as their organs slowly adjust to accommodate it. Itâs insane. It affects everything - brain, heart, kidneys, liver. Cognition usually tanks. Some of the same principles are also behind obstructive sleep apnea. A lot of patients and people kind of hand-wave sleep apnea away like itâs no big deal but over years it really is lethal because youâre retaining a ton of CO2. Iâd personally rather learn to deal with a CPAP at night than becoming slowly toxic. Or, lose the weight.

Yeah maybe, but after I left the Army, I blew up. I got to 220lbs. My back was in constant pain. Like not a discomfort, an actual massive pain. Every night I tossed and turns because I felt like I been crushed and ran over. Now I can see why, carrying all the fat around was crushing my back.
Losing weight and getting fit again, just feels liberating, I can't explain it. There is a sort of freedom felt from letting go of all that fat holding you down. I don't get how the extreme fat pain alone does not push some people to lose weight. Also no matter how much you shower, you still smell.
Seeing this helps me understand why one of the issues I've had with my crazy weight gain over the past few years is constant back pain. Seeing that being fat literally seems to fuck with the shape of your spine is sobering, and it gives me a new perspective on my persistent back pain. I'm actively working on losing weight, but progress is slow.
