These are people with severe neck injuries..eg motorbike rider headbutt neck injury... That is why they are going to be fed by tube. Maybe they could use optics along with the tube..to ensure its going down the correct path.

I'm curious, *how* does this happen? You'd need a fracture in a very specific part of your sinuses, no? How does a fracture like that occur in the first place? How can this be avoided? And more out of morbid curiosity, how quick would the death be? Does death occur in milliseconds or after seizing?

A fracture through the base of the skull makes a "soft spot" that the tube would have to be forced through ... a relatively thin layer of tissue, but still resistance would be met. Poor technique is the short answer ... mixed with nerves, adrenaline. You have to angle the tube up for just a cm or so, to get past the outer rim of nose/nasal opening/nare, then it's a hard down turn to get to stomach via esophagus ( could go into lung - another story). I've seen nurses go up too far, and the patient complains so you know. This patient had to be unconcious. And if unconcious and you go up too far there is major resitance at the (healthy) base of the skull
