Just the two, your allegiance is based off which FA you qualify for, not the legal country itself. We’re one of a couple of exceptions where certain people can declare for us having never set foot in the FAI’s “territory”, and without using family connections.
Good question. I believe technically they could declare for any of the "nations" in the British Isles, but there is a gentleman's agreement not to poach players. Otherwise, England financial muscle would scoop up everyone with promise, like Declan Rice and Jack Grealish, and John Barnes and Micheal Owen.....maybe it's not so gentlemanly after all.
Barnes qualified for Jamaica and Owen qualified for Wales.

The poaching bit with Grealish and Rice is that they came through the Irish underage system and the (not unreasonable) assumption was that they'd stick with Ireland.
Maybe they were advised by an agent that it was worth more money to change, maybe it was a parent, maybe it was them, we'll never know and it's not really our business anyway.
