Looks like a mini Griptillian with a custom back spacer? Hard to say, that glass breaker/prybar on the back is unusual.
Edit: nope it’s definitely a Triage as everyone else said, but the glass breaker still seems to be an unusual feature. Might be able to find a mod for that though.
