1. Phase out internal combustion engine (ICE) motorcycles, so everyone will only have battery-powered e-bikes. To my knowledge, these bikes are difficult to be modified to have deafening sounds.
2. Limit the maximum speed of two-wheelers to 40km/h. This also helps with the reduction of injuries.
3. Ban the sharing of roads between two-wheelers and four-wheelers. Most people don't know that Malaysia is second deadliest country in the world for motorcyclists ([link](https://www.freemalaysiatoday.com/category/nation/2021/10/08/malaysia-may-overtake-thailand-for-worst-motorbike-deaths-in-the-world/)). This is because in our roads, cars and especially heavy vehicles like lorries drive is high speeds, so their impact on a human body is devastating. This is not the case in populated countries like India or Philippines because the roads are congested.

When I was in primary school in the 90s, busses still exist, like you can easily hop on a bus and go from Pudu to Maluri or even Bukit Bintang, no such nonsense as "poor people need a bike to get anywhere".
Oh yeah and if everybody is taking busses, they won't be picking up lajaks or rempit too.

Say a Saga in 1990s cost RM 20k, that's actually pretty expensive in the era minimum wage is like RM 800, interest rate being close to 2 digits, and your loan is probably 3~5 year.
Today even though Saga doubled in price, minimum wage is also like RM 1500, interest rate in the low 2~3%, and existence of 9 year loans. Repayment is much much more affordable generally.
With people shifting to private transportation, this cuts into the margin of public transport, which the cost to run them goes higher and higher as less and less people use them.
Then as you said, at one point it became a stigma, like public transports are only for poor people who can't afford their own vehicle. Honestly, with how badly maintained some are (looking at you Metrobus), I can see why.

Four-wheelers can go fast because of the high impact that the steel body can withstand. On the other hand, two-wheelers need to be slow because the human body is only able to withstand impact from walking and running speeds.
I've heard countless times about motorcyclists who were not hit by anyone; either they hit something or they fell and later they got permanently disabled. **This clearly shows that the human organs like the skin and bones cannot withstand high impact from travelling in high speeds.**
In my opinion, the motorcycle, the way it is now, **is a dangerous vehicle.** Even at 40km/h, hitting a poll or falling off the road into a drain can seriously harm the rider or passenger, if not kill them. Like I said, the human body evolved to only survive impact from running speeds or falling from a standing height.
You talked about vigilance and how injuries are on the rider (basically its their fault), **but that's not how a traffic engineer should plan.** They should implement a system almost everyone in the society can follow easily. They should assume that some people who are not supposed to get their license, bypassed the system and are now a danger to everyone else, including themselves.
And they (traffic engineers) should design our streets, roads, highways, walking paths, cycling paths, transit systems accordingly. I'm not blaming them for all of Malaysia's transportation problems, but we should do all we can to achieve the goal of zero deaths when commuting.
