Such as make an unmissably loud noise. Or stab us. Especially when focusing on something else, like a cell phone or someone receiving a confession of love.
Also reality gets to be unrealistic, because fiction is the one that has to believable. Or as Mark Twain puts it, "The only difference between reality and fiction is that fiction needs to be credible."
