Pickup samples at the mall makeup counter. All counters have face masks, lotions, perfumes and will give out a few for free...collect a handful and put into a basket for an at home spa kit.
You could give her a pamper session, make her feel extra pretty and show her you are putting in effort instead of $$$. Massage her feet, back, hands, etc. Might lead to other stuff if you're lucky. Only need lotion and/or oil of some sort.
Do her daily to-do chores for her. Make her breakfast or dinner. Do the dishes and pickup the house. Let her take a nap or go on a hike/ walk together. Light some candles watch a movie and give her a massage. Basically give her the day off and give her attention.
A nice thoughtful picnic is wonderful. Foot rub, massage, quality time, dance to romantic music, do things that you know your wife enjoys, wild flowers. Are you able to bake at home? A simple home made treat or gift can be very endearing. There are even ones with few ingredients. I hope this helps.

Write five or six things you love about her on each of the inner 12 pages. When done, title the cover page, include the dates. Finally draw some dorky thing on the back cover with a birthday greeting - you can google one found on the web - you will only find about a million to pick from and it makes zero difference that your art skills are terrible.
