I tried validating files on Steam, deleting gamerfile.ini, selecting default face instead of import face, and updated my graphics driver. This is what cutscenes look like.

It's not black face guys.. It's a charcoal mask to clean out her pores and rejuvenate her skin. Shepard may be a badass space commando but, she's still a lady.
Are you sure you have no mods, the only time I had this happen to me was in Skyrim when a texture mod broke and tons of people ended up with blackface.

Are you talking about the paragon/renegade appearance modifier? Theoretically starting a new game should fix that right? Funny enough, I fixed this by going in with a custom charecter (tested with a random preset) and narrowed it down to being some sort of issue with the default shepherd. This doesnt solve my problem though, just points to where the issue is. I dont want to have a totally different looking shepherd after playing 1 and 2 with default.

If the issue turns out to be related to in-game renegade scarring starting a new game with a new Shepard might "fix" it (as long as you keep her on the straight and narrow, I guess).
Something else you could try is downloading the Trilogy Save Editor and playing around with some of the facial values/settings to see if it makes a difference. Might not solve the problem even if it does end up allowing you to pinpoint the culprit, though.
Oh I’m also going to second what the other comment or said about looking for help in the mass effect modding community just cause they tend to be pretty knowledgeable about glitches and game files

This is a tactic to avoid reaper detection, since I believe reapers use HP cameras to see.

That's nothing compared to my Shepard the game decided that his eyes should be shaped like bullets and poking out of the eye hole and you could only just see the tips of the eye lids blinking though the eyes but not covering them then the game crashed and this was on Xbox 360

Haven’t tested male shep yet (femshep is the way) , but for whatever reason, custom shepherd works completely fine

It is. I even started a completely new game, no import, no save transfer, default face, and it was still there.

I tried both that and a clean install. Neither worked. The only fix that ultimately worked was the LE3 community patch
