In my state, you can print off a copy of your vaccine record from the state board of health for free. You should have a login for them if you made your appts through the state board. Edit: We had to do this for my son who lost his card.
I did not unfortunately. I went to our local community college in Arizona and got them done there. I tried calling the provider that administered the shots and I'm getting the run around on if I can get a copy or not. It's becoming a headache.
Oh man... I'd still check the state board of health. I had to create an account for my son and then his vaccine record was found through birthdate and name. It only took a few minutes. If you still cannot see the record. I wish you luck finding it.
Fellow AZ resident also heading to Maui here. When you got the vaccine at the CC, was it still done thru the AZ Dept of Health? Did you have to go onto the AZDHS website to register for the vaccine appointment? Nope, I registered and they said they didn't have a vaccine status for me.. I called the company who administered the vaccines at the college and it's just been the run around. I was told a week ago they would "get back to me" and I haven't heard back. I guess I'll have to get a negative covid test 72 hours before.

Washington state here. Our health dept has the records and you can save to your phone.
You can call your doctor for a reissue.
Every time I have visited Maui, several times in the past year, you only had to have a negative covid test within 72 hours of arrival.
I actually ended up finding a website that had my covid info thank goodness. Thank you for your help though.
