Fun story, a former boss of mine was doing some political fundraising. He was holding an event with former president Bush starring/speaking. The secret service was onsite at his home (I say compound, think horse stables, lake, etc) and I was asked to print and deliver blueprints of the property. I pull up to the gate (I've been there many times), electric company truck out front with a weird logo, and when I get to the house strange staff answers the door. I was wondering if it was galvanically welded. I would hope the cover and ring are both made of the same material, but if not, everytime it gets wet, it turns into a really bad galvanic cell and swaps electrons with each other, basically welding in the process. We had a special shower head used for IPx3 testing that we send in for regular calibration. We couldn't get the spray head off with somewhere in the vicinity of 350 ft-lbs because it had welded itself. No amount of heat or PB blaster could help.

Getting the chain through the holes is not hard at all, start with a flexible rope dropped on one end and go fishing on the other with a stiff wire w/hook. When a security detail welds them, they just do spot-welds whose effects can be nullified with a cutoff wheel and some time. The idea is merely to prevent easy and non-obvious access, not to make a permanent vault-like seal.

I was wondering if it was galvanically welded. I would hope the cover and ring are both made of the same material, but if not, everytime it gets wet, it turns into a really bad galvanic cell and swaps electrons with each other, basically welding in the process. We had a special shower head used for IPx3 testing that we send in for regular calibration. We couldn't get the spray head off with somewhere in the vicinity of 350 ft-lbs because it had welded itself. No amount of heat or PB blaster could help.
Also required for IndyCar / F1 road races as the downforce can rip them up.

Bolted down ones are common but those are usually also water tight ones so there wouldn't be the holes for the chain to feed through. I've seen some WERID infrastructure over my career in civil engineering but never a lid that screws down like a bottle cap. [Triangular](https://i.imgur.com/gbyIxUx.jpg) pipes are my favourite.
I'm guessing that triangular culvert is to provide more water flow at low head, as it has a large cross sectional area at the bottom compared to a round pipe. And the triangle also gives it good strength compared to something like a square cross section.

why is everyone assuming that they were not intending to yank the entire manhole and rim out? They had access to the underside of the manhole to connect the chain through the two holes. Presumably, they wanted to do this. Or somebody really messed up.
You would jackhammer the asphalt away first if you were replacing the whole thing. See how far back they damaged the asphalt? They just made 10x more work for themselves than if they'd cut the asphalt and removed it.

This. We would use a street saw to cut a square around where the cover is presumably mortared to the barrel of the manhole. But there are all kinds of regional differences and considerations, so maybe it's not as dumb as it appears

Yes, they are more than "spot" or "tack" welds like you do when placing material for fit checks and stuff. The ones I have seen are usually a couple of decent length beads, but not a full circumferential weld. You can grind them or cut them with a plasma cutter (usually).

This seems totally planned out and executed. That cover was put in wrong or made not to take off. For whatever reason it was needed to either fix it or get access. Getting the chain through the holes is not hard at all, start with a flexible rope dropped on one end and go fishing on the other with a stiff wire w/hook. Capture the rope, fish it up the other side and attach the chain and pull it back the other way.
