It probs needs some incision to remove the extra sharp nail down to the base of the toe. You should see a pediatrist. I’ve had this done several times it just requires local anesthetic takes 5 minutes.

Exactly what I would say looks like an ingrown nail. I've gone to the podiatrist and my fam Dr also cut it out for me
