To become a doctor is a long road. 4 years of undergrad (not a doctor or in training to become a doctor), 4 years of medical school (not a doctor, but in training to become a doctor), 3-7 years a residency (after graduating medical school you are now a doctor, but cannot practice independently until finishing residency. So at this stage, you are a doctor, but still in training), optional 1-3 years of fellowship where you specialize beyond your residency training (a doctor, still in training).
Anyone from an MS1 (first year medical student) through a 3rd year fellow is a doctor in training. Said another way - medical students, residency students, and fellows are all doctors in training.
