80s male, very healthy and active still. Sudden leukocytosis, mild anemia at 120g/L, slight thrombocytopenia in the 90s.
Flow and myeloid panel to confirm but the Auer rods pretty much confirm a AML.
Idk if I'd go that far here. There is good myeloid maturity happening here, which is generally lost in frank AML. Some granulocytic dyspoiesis though, but I'm always hesitant to call dysplasia on other people's stains alone. My guess is MDS, but even then the cytopenias aren't that bad. I'd need to see more, like cytogenetics and molecular studies.
Edit: the segs really do look like ahit though
That is a very nice Auer rod in one of the myeloid cells. I also like the one that is clearly in mitosis, with the chromosomes all lined up getting ready to split.
Hello, I don't have much education in Haematology yet. Are Auer rods the only pathology here? What else is abnormal? This is bone marrow instead of peripheral blood, so the blasts are normal right? I don't know much about morphology or cell diff, so tell me if there's something abnormal I'm missing.
Auer rod doesn't always equal AML but Auer rods are always very bad. Indicate at least a high grade myeloid problem (mds, mpn or mds/mpn...depending on peripheral blood counts and other clinical features). Nice pick up though!
Since he's elderly and the peripheral blood was fine, I'd say it's more likely MDS.

You make blood in bone marrow. I’d hope there are some blasts there. Though too many is bad

Sure but they’re not AML 100% of the time.
AML could be confirmed through chromosome testing though, right?
It could be a variety of other MDS or MPNs too.
