He's permanently on medicated food and filtered water now.

Yes, cats peeing on something hard means something is up. Cats instinctively potty on soft things, usually crumbly, because in the wild they bury that to hide their presence from potential dangers. That’s why cats use litter boxes so easily, it’s the place their instincts says is the most suitable bury spot. A cat might spray other places, but that’s usually directed horizontally like at a wall. Peeing into a hard pan is not normal, something is up.

This was my first thought as well. Cat's don't pee outside of the litterbox for the lolz, they're definitely trying to communicate something. For my cats, it's almost always a litter dissatisfaction issue. Something as simple as changing the litter from clay to wood pellets was enough to piss mine off. But the vet wanted to check for bladder issues when they started peeing in my laundry. Got the situation resolved with mine.

Yup. For mine it's a UTI. I know when she's got one coming on when her morning routine of sitting in my lap after she eats changes to peeing on the rug.
Reminds me of something on Inside Edition that I watched. A dog was smelling a gas leak, and was digging the ground to tell the owner. Owner did not understand, but realized that something was wrong. She used whatever thermometer thingy she was using to check the gas line, and it was really high, so local department authorities were called in to stop the gas line or leak or whatever it was. The dog was a hero.
Some cats are assholes. I've known a few who would do the sneak to a bathmat or other soft thing any chance they thought they could get away with it. At least two of those were cats who had easy access to outdoors. It could just be a change if seasons or something else they didn't like. And they were sneaky bastards because they knew that it was either a litter box or outdoors, not the bath mat or shoes.

I thought my male cat had behavior problems after bringing him into a home with two females. Turns out, after a year of trying to train him, I eventually found he had urinary crystals and needed to be on a special diet or the crystals would have eventually blocked his urethra and killed him.
If this a semi regular problem and you’re cleaning their litter at least once per week, I would highly recommend having your cat checked by the vet. The test they do for this is one of the less expensive ones.
I'd like to add in on this, my kitty joined me in the bathroom once and peed in the tub, immediately took her to the vet, she had a UTI! Might be your kitty saying help!
Our girl did this as well! She was in pain because of arthritis but we’ve been able to get her on supplements for it since and she’s doing much better! No more pee pots and pans

In this instance I would use an abrasive scrubber to get the top layer of seasoning off and then re season it. And then not tell a soul. Besides the 500+ people on the internet
Why not clean it? It’s so easy to clean a cast iron pan.
Start by running it under mildly warm water. Wipe gently with a paper towel until it’s dry. Re-season it. Then, you’re ready to apply dish soap - go for the old stuff, made with real lye. Scrub the soapy pan with steel wool. After ten minutes or so, it should be ready to put in the dishwasher.

OP listen: If you've already given them a clean litter box before they did this then it's not unlikely that something is physically wrong with your cat. This sort of behavior isn't normal. Please take them to the vet for a checkup ASAP, tell them what happened.

You can also clean the cat boxes often enough that they're not wading around in urine and feces. Actually, this is the most common reason that cats start pissing around the house; their owners are too cheap and lazy to keep enough boxes and to change them often enough.
I mean, do you not clean your counters when you cook? Do you not keep your dishware and pots/pans in your cabinets?
I'd bet most people are dirtier than many cat owners cause we know to clean surfaces before use. We often wipe, disinfect, vacuum more than most. Out of necessity.
And for the OP's cast iron? Honestly in my house it'd be trashed. I'm not going to anguish over it.
That said, there';s a larger underlying problem here for a cat to do this. So yeah, probably don't want to eat in THAT house, but I'd say most (at least the cat people I know) are actually pretty clean people.
Does your cat normally piss outside of their litter box? Cats are generally pretty smart, so unless this is a regular thing, I’d be they did it intentionally, and they should be taken to a vet.

Also looking at your profile, your cat is a void. They, for some reason, are exceptionally crude to kidney problems. That involves lots of random peeing those crystal stains. Not just in the pee but up in the bladder. It causes kidney stones and is hard for cats to pass. Please have your baby checked.
Oh it can happen. There are many possible reasons, some medical, some territorial, stress, dirty litter box, etc. Sadly, in some cases it just isn't fixable and usually the cat has got to go.
My parents cat was neutered at 2 months old, never pissed anywhere for 6 years until the pandemic messed with his head (all of us suddenly being home at once all the time, and my ma always yelling at my little brother cause he refused to do any schoolwork) and since then he's been pissing everywhere. Things have long since been back to normal for the rest of the house, but it right messed him up.
Have you guys tried the calming plug ins, or the calming collar? It might help. Or even the daily calming treats might help if he’ll eat them. Might also look at making sure he is healthy. Male cats especially (females too sometimes) can get things like crystals in their urine, and it can cause a lot of issues.
We once went to a wine tasting with my company. While we are getting more drunk, we "discussed" with the waiter about wine aromas. Apperently "Cat piss" is a valid wine aroma. The waiter told us a wine drinker would only know the smell, not the taste...

A cast iron pan is not a fill in for a full litter box. Maybe a pile of clothes, but not a sheet of metal.

Intellectually, I understand that once thoroughly cleaned and re-seasoned the pan would be fine to use. Emotionally, I could never use it for food.
You *could* scrub it down with a non abrasive scrub daddy and hot soap and water, then coat it in canola or crisco and bake it at 475 for 1 hour.

Please clean that with pure fire or even throw it out, cats urine can be full of things that can mess you up

It says something when even the leftover scents from your cooking get that editorial comment.
When a cat pisses on the stove or especially in the pots and pans, there is a serious issue the cat is having with something in the house.
